两种不同治疗方法对老年性阴道炎的复发疗效比较

摘　　要：目的:探讨甲硝唑+乳酸菌与甲硝唑+雌三醇两组治疗方法,对治疗老年性阴道炎的临床效果。方法:选择2011年1月～2012年12月在本院妇科门诊就诊的老年性阴道炎患者100例,随机分为两组:因中途1例退出雌三醇治疗,即观察组为49例,对照组为50例。两组阴道给药,开始均使用甲硝唑片0.2g/次,1次/天,连用7天,观察组在甲硝唑治疗结束后用雌三醇乳膏0.5g/次,1次/天,连用14天,之后改每周2次,维持治疗8周;对照组在甲硝唑治疗结束后用乳酸菌胶囊0.25g/粒,2粒/天,连用14天,之后改每周2次,维持治疗8周。比较两组临床应用效果。结果:①在阴道用药3周后,观察组和对照组的总有效率均为100%。②在治疗后第2、3个月对两组患者随访时复发率均为0%,在第4、5、6个月时,观察组复发率分别为:4.08%、8.16%、20.41%,照组复发率分别为:20.00%、26.00%、42.00%,两组间比较差异有统计学意义(P=0.03、P=0.04、P=0.04)。③观察组有4例患者出现乳胀及阴道灼热感,对照组无不良反应。结论:雌三醇乳膏在预防老年性阴道炎复发方面优于乳酸菌胶囊。在用药安全性方面乳酸菌胶囊更胜一筹。Objective： Estriol two group therapy,in the treatment of senile vaginitis clinical effect.Methods：2011 January to 2012 December in our hospital gynecological clinics in 100 cases of senile vaginitis patients,were randomly divided into two groups（except for one case quit）： the observation group（49 cases） and control group（50 cases）.The two groups were vaginal administration,began using metronidazole tablets 0.2g,1 times a day,for 7 days,in the observation group after treatment with metronidazole estriol cream 0.5g,1 times a day,for 14 days,then 2 times a week,maintenance therapy for 8 weeks;in the control group after treatment with metronidazole Lactobacillus capsule 0.5g daily,for 14 days,then 2 times a week,maintenance therapy for 8 weeks.Comparison of two different treatment methods in clinical application effects.Results： ①In the vagina after treatment for 3 weeks,the observation group and the control group in the total efficiency was 100%.② 3 months after treatment in two groups of patients followed up at the time of relapse rate was 0%,4,5,and 6 months,the observation group recurrence rates were： 4.08%,8.16%,20.41%,as the control group of recurrence rates were： 20%,26%,42%,the difference between the two groups was statistically significance（P= 0.03,P= 0.04,P=0.04）.The observation group has 4 cases with breast swelling and vaginal burning sensation,no adverse reaction occurred in the control group.Conclusion： Estriol cream in the prevention of senile vaginitis recurrence than Lactobacillus capsule.On the safety aspects of Lactobacillus capsule is more than estriol.
