摘 要:目的评价牙科显微镜结合超声技术处理根管内折断器械的临床效果。方法 38个根管内的折断器械按种类分为不锈钢挫、镍钛挫、根管桩,应用显微超声技术处理折断于不同位置的不同器械并分析其结果。结果 38个根管取出27个,总成功率71.1%。其中不锈钢挫成功率71.4%,镍钛挫成功率37.5%,根管桩成功率100.0%。无一例根管侧穿。按折断器械位置分,不锈钢挫在根上1/3取出率为100.0%,根中1/3为81.8%,根尖1/3为33.3%;镍钛挫在根中1/3取出率为60.0%,根尖1/3取出率为0。在弯曲根管中,不锈钢挫在弯曲冠方取出率为86.7%,弯曲根方取出率为20.0%;镍钛挫在弯曲冠方取出率为60.0%,弯曲根方为0。结论牙科显微超声技术对根管内折断器械的取出是一个行之有效的方法,但对于镍钛器械和折断于根尖1/3或根管弯曲根方的病例取出率相对较低。Objective To evaluate the clinic effects of removal of fractured instrument in root canal by dental microscope combined with ultrasound technique. Methods The fractured instruments in 38 root canals were divided into stainless steel file,Ni-Ti file, and root canal pile according to their kinds. Microscope and ultrasonic technique were used to take out the fractured instruments in different sites, and the results were analyzed. Results Among the 38 root canals ,27 ones were taken out, and the total success rate was 71.1%. The success rate for stainless steel file removal Was 71.4% ;for Ni-Ti file removal was 37.5 % ;and for root canal pile removal was 100.0%. No root canal perforation occurred. According to the site of fractured instrument, the removal rate of stainless steel file located at upper 1/3 of root canals was 100.0% ;at middle 1/3 was 81.8% ;and at 1/3 apex was 33.3%. The removal rate of Ni-Ti file located at middle 1/3 was 60.0%, and at 1/3 apex was O. In the bent root canals,the removal rate of stainless steel file located at bent part crown square was 86.7%, and at bent part apex square was 20.0%. The removal rate of Ni-Ti file located at bent part crown square was 60.0%, and at bent part apex square was 0. Conclusion Dental microscope combined with ultrasonic technique is an effective method for the removal of fractured instruments in root canals. But removal rate is relatively low for Ni-Ti instruments and those fractured instruments located at 1/3 root canal apex or bent part apex square.
