水平斜视术后屈光状态的临床研究

摘　　要：目的 :了解斜视术后眼球屈光状态的动态变化及不同术式对屈光状态的影响 ,并对其发生机理进行初步探讨。方法 :用散瞳验光和角膜地形图检查 ,对 6 6例水平斜视患者手术前后屈光状态进行动态分析。结果 :各组的角膜曲率及dk值均差异有显著性 (P <0 .0 5) ,散光组与非散光组dk值变化平均值比较 ,差异有显著性 (P <0 .0 1)。Ⅱ ,Ⅲ 和 Ⅳ 组球镜屈光度术后差异有显著性 (P <0 .0 5) ,Ⅱ ,Ⅲ 和 Ⅳ 组柱镜屈光度术后差异有显著性 (P <0 .0 1)。结论 :水平斜视手术对眼球屈光状态的改变主要是对角膜的影响 ,且是可恢复的 。Objective: To find out the dynamic refraction changes following horizontal strabismus surgery and to study the effect of different surgical procedures on refraction and its mechanism. Methods:The refraction conditions of 66 cases of horizontal strabismus were analysed by cycloplegic refraction and topography. Results:There were significant differences in corneal curvature and dk value in all groups (P<0.05),significant difference in dk value between astigmatic and non astigmatic group(P<0.01) and significant differences in spherical diopters and cylindrical diopters in group Ⅱ,Ⅲand Ⅳ (P<0.05) postoperatively. Conclusion:The changes of refraction condition through horizontal strabismus mainly have effect on the cornea,but they are reversible. The recovery time varies with different procedures.
