血清可溶性上皮型钙黏附蛋白与胃癌侵袭转移的关系

摘　　要：目的:探讨血清可溶性上皮型钙黏附蛋白(soluble epithelial cadherin,sE-cad)的含量变化与胃癌侵袭转移及复发的关系。方法:以ELISA法检测158例胃癌患者及30例正常对照者血清sE-cad与CEA的含量,并检测胃癌患者术后1个月sE-cad与CEA含量的变化。术后随访3年,肿瘤复发患者复查血清sE-cad与CEA含量。分析血清sE-cad含量变化与胃癌病理特征的关系。结果:胃癌组和复发转移组血清sE-cad、CEA含量均显著高于正常对照组(P<0.01)。术后1个月sE-cad含量降至正常范围(P﹥0.05),而CEA含量仍显著高于正常范围(P<0.01)。血清sE-cad含量与肿瘤分化程度、TNM病理分期、有无淋巴结转移、有无远处转移、浸润深度均相关(P<0.01)。结论:血清sE-cad可成为具有一定临床价值的新型胃癌肿瘤标志物,有助于术后复发的监测和预后评价。Objective To study the relationship of serum level of soluble epithelial cadherin（sE-cad）with the recurrence,invasion and metastasis of gastric cancer.Methods Serum levels of sE-cad and CEA in 30 normal controls and 158 patients with gastric cancer before and 1 month after surgery were measured by ELISA.After 3 years of follow-up,serum levels of sE-cad and CEA were also measured in the recurred gastric cancer patient.The relation between serum level of sE-cad and the pathological characteristics of gastric cancer was evaluated.Results Serum levels of sE-cad and CEA in gastric cancer group and recurred and metastasis group were significantly higher than those in control group（P〈0.01）,and sE-cad level was decreased to the normal range 1 month after surgery（P〈0.05）,while CEA level was still significantly higher（P〈0.01）.Serum level of sE-cad in gastric cancer group was significantly correlated with the differentiation degree,the pathological staging,the lymph node metastasis,the distant metastasis,and the depth of invasion（P〈0.01）.Conclusion Serum level of sE-cad could be a valuable gastric cancer marker and conduce to monitoring the recurrence and to evaluating the prognosis of gastric cancer.
