摘  要:目的:从解剖学角度探讨人类胚胎睾丸下降机理以及隐睾的发生原因。方法:解剖22周以上流产死胎15具以及成人尸体标本14具,观察鞘突、鞘膜囊开放、充盈状态,测量内环日与腹股沟韧带、耻骨结节之间的距离和角度,对照20例隐睾手术时的测量数据进行统计学处理。结果:发现胚胎期睾丸鞘膜囊均为积水膨大状态,鞘突开放,内环口全部位于腹膜腔生理性积水之下,隐睾病人的内环位置偏高或偏内,与胚胎和尸检相比有明显差异(P<0.01)。结论:睾丸的下降过程可能与开放的鞘突和鞘膜囊、腹压有关,而腹膜腔内的生理性积水则有可能起到了传导压力推动睾丸下降的重要作用。隐睾的发生机制可能与内环口的位置异常,胎儿腹腔生理性积水过少,睾丸引带缺如或异位附着有关。To explore the mechanism of testicular descent of human fetus and the cause of cryptorchism. Methods:Fifteen dead fetuses by abortion aged more than 22 weeks and 14 adult cadavers were analyzed by autopsy in this study.The open or engorged state of vaginal process and tunica vaginalis was observed and the distances and angles of the intend ring orifice to inguinal ligament and pubic tubercula were measured All these data were contrasted with the data obtained from operation of cryptochism. Results: The tunica vaginalis in nrmal fetus was found in a hydropsy condition, and vaginal process was opened. All internal ring orifices of the specimens were found tinder the level of physiological hydropsy in peritoneal cavity. The position of internal ring in cryptochism cases was much higher or interior than those of normal fetus or adult cadavers (P < 0.01). Conclusion: The testicular descent was probably related to the opening of vaginal process and turica vaginalis or abdominal pressure. The physiological hydrosy in abdomen may play an important role in pressure conduction to motivate the testicular descent. The pathogeny of cryptorchism may be related to the abnormal position of intend ring, less physiologic hydropsy in fetus abdomen and absence or ectoic adhesion of gubernaculum testis.
