摘　　要：目的 探讨人脐血单个核细胞（HCMNC）移植改善老年人血管性痴呆（VaD）患者神经功能、认知功能和日常生活活动能力情况及其安全性.方法 选择2005 年4 月至2009 年12 月在郑州市第二人民医院住院和门诊收治的32 例VaD 患者,均经CT、MRI检查确诊,符合第四届全国脑血管病学术会议修订的脑卒中诊断标准.采集足月新生儿脐带血,无菌血袋分离出单个核细胞,通过手背浅静脉将HCMNC 输注入脑卒中患者体内,每例患者平均输注6 份（细胞数≥ 1×109/ 份）,每份间隔时间1-7 d,平均4 d.治疗前后神经功能缺损（NFD）、简易智能量表（MMSE）和日常生活能力（ADL）的比较采用配对t 检验.结果 32 例患者,治疗前与治疗后6 个月临床神经功能缺损程度评分分别为（23.093 ± 6.907）分,（12.938 ± 4.500）分,差异有统计学意义（t = 11.648,P = 0.000）;MMSE 评分分别为（13.281 ± 3.466）分,（23.219 ± 3.087）分,差异有统计学意义（t =18.844,P = 0.000）;巴塞尔指数（BI）评分分别为（28.125 ± 15.644）分,（64.531 ± 14.555）分,差异有统计学意义（t = 16.794,P = 0.000）.在整个治疗过程中全部患者均未使用免疫抑制剂,未发生并发症及不良反应.结论 多份HCMNC 手背浅静脉移植法简单、安全,可作为治疗老年人VaD 的有效手段之一.Objective To investigate the efficiency and safety of human umbilical cord blood-mononuclear cell transplantation for patients with vascular dementia. Methods Thirty- two patients with vascular dementia were enrolled at the Zhengzhou Second People＇s Hospital from April 2005 to December 2009. They had been diagnosed with computer technology （CT） and magnetic resonance imaging（MRI）. The diagnostic for apoplexy of brain was made according yo the fourth national academic meeting of cerebrovascular diseases of China. Umbilical cord blood was collected from the mature delivery. Mononuclear cells were harvested using the aseptic collection bag, and then injected into stroke patients via superficial vein of the back of the hand, 6 times for in each patient（cell number ≥ 1×10^9 per sample ）, with an interval of 1-7 days. Results Compared with pretransplantation, scores of neurologic impairment degree were significantly decreased in patients 6 months after transplantation （t = 11.648, P 〈 0.01） Mini-mental state examination （MMSE） and Barthel index grades were significantly increased （ t = -18.844, -16.794, P 〈 0.01）. No immunosuppressant was used. Complication or adverse reaction was not seen following transplantation. Conclusions HCMNC transplantation via superficial vein of the back of the hand is simple, safe and effective for vascular dementia.
