摘 要:目的分析宫颈上皮内瘤变(Cervical intraepithelial neoplasia,CIN)由低度病变、高度病变、宫颈癌三个病程中细胞免疫的变化。方法采用流式细胞仪检测58例低度病变,37例高度病变,宫颈癌21例患者T淋巴细胞亚群中的CD4+、CD8+、CD16+56+的活性细胞数量及比值;用ELISA方法检测患者血清中IL-2、IFN-r和IL-10因子水平,30例健康体检者作为对照组。结果低度病变的免疫细胞数量与对照组比较,CD8+、CD4/CD8比值无显著差异(P>0.05),免疫细胞的IL-2、IFN-r因子水平与对照组无显著差异(P>0.05);其他均与对照组有显著差异(P<0.05;P<0.01);三个病程阶段之间相比较,从低度病变到高度病变再到宫颈癌整个病程中,低度病变CD16+56+细胞数量增加,从高度以上病变开始数量逐渐减低;CD4+、CD8+、CD4/CD8、IL-2、IFN-r的活性水平逐渐下降,而IL-10水平逐渐升高,三个阶段的免疫活性细胞数量及因子水平有显著差异(P<0.01)。结论实验研究表明宫颈上皮内瘤病变由低度向高度病变及癌变病程中的CD4+、CD8+、CD16+56+、IL-2、IFN-r、IL-10这几项指标之间相互影响制约,提示患者CIN与宫颈癌的病程发展与机体免疫细胞数量以及细胞因子活性水平调节有关。Objective To investigate the pathological changes of cell immune in cervical intraepithelial neoplasia (CIN) from lowere lesion to high lesions. Methods The number and ratio of T lymphocyte subsets CD4 +, CD8 +, CD16 + 56 +in lower lesions of 58 cases, 37 cases of highly lesions, 21 cases of cervical cancer patients were detected by flow eytometry. The levels of serum IL-2, IFN-r and IL-10 were determined by using ELISA, 30 healthy persons were as control group. Results No significant differences in the quantity of immune cells and the ratios of CD8 + and CD4/CD8 in the patients with lower lesions was observed compared to the control group (P〉0.05), so was the levels of IL-2 and IFN-r of the immune cells compared to the control group (p〉0.05 and P〈0.01). Then number of CD16+56+ during the course of from lower lesion to higher lesion and to cervical cancer was increased, the number of CD16+56+ during the course of from higher lesions and cervical carcinoma were gradually decreased; the activity levels of CD4+, CD8+, CD4/CD8, IL-2 and IFN-r was gradually decreased, while the level of IL-10 was gradually increased showing significant differences in quantity of immune cells and levels during the three courses (P 〈 0.01); Conclusion The levels of CD4 +, CD8 +, CD16 + 56 +, IL - 2, IFN - r and IL-10 in the patients with cervical intraepithelial neoplasia during the course of from lower lesions to higher lesions were interacted, suggesting that CIN and the development of cervical cancer were associated with the quantity of immune cells and the regulation of active level of eytokines.
