八宝丹辅助羟基喜树碱膀胱灌注预防浅表性膀胱癌术后复发的疗效分析

摘　　要：目的评价八宝丹辅助羟基喜树碱膀胱灌注预防浅表性膀胱癌术后复发的临床疗效及安全性。方法回顾2008年6月至2010年6月我院收治的56例浅表性膀胱癌患者，均采用保留膀胱经尿道膀胱肿瘤切除术。依治疗方法不同分为观察组和对照组，各28例。观察组术后行羟基喜树碱膀胱内灌注预防肿瘤复发辅助口服八宝丹胶囊（0．6g／次，3次／d），对照组28例术后仅行羟基喜树碱膀胱内灌注治疗。在2年随访期内观察患者肿瘤复发情况及药物不良反应。结果对照组2年内复发8例，复发率为28．6％；5例药物不良反应发生，发生率为17．9％。观察组2年内复发7例，复发率为25．0％；3例药物不良反应发生，发生率为10．7％。观察组2年复发率及不良反应发生率均低于对照组，但差异无统计学意义（P〉0．05）。结论八宝丹可以辅助羟基喜树碱膀胱灌注预防浅表性膀胱癌术后复发，并可能降低化疗药物膀胱灌注的不良反应。Objective To study the effect of Babaodan auxiliary hydroxycamptothecin on preventing the re- currence of postoperative intravesical therapy in superficial bladder tumor. Methods The clinical data of 56 postop- erative patients with intermediate and high-risk urothelial tumor of the bladder treated with keep transurethral resec- tion of the bladder tumor were reviewed as observation group. The outcome of Babaodan auxiliary hydroxyeamptothe- cin preventing the recurrence of postoperative intravesical therapy after operation was observed. Twenty-eight eases with only line hydroxycamptothecin bladder filling were selected as control group. The tumor recurrence rates and side effects after the treatment were evaluated for 2 years. Results Eight cases had recurrence in the control group within 2 years and the recurrence rate was 28.6% ; five cases had adverse drug reactions and the incidence rate was 17.9%. seven cases had recurrence in the observation group within 2 years and the recurrence rate was 25.0% ; three cases had adverse drug reactions and the incidence was 10.7%. The 2-year recurrence rate and adverse drug reactions incidence in the observation group were lower than the control group, but the difference was not statistically significant （ P 〉 0.05 ）. Conclusion Babaodan auxiliary hydroxycamptothecin emulsion after operation is safe and may be effective in preventing superficial bladder tumor recurrence.
