摘  要:目的:探讨64排螺旋CT对胃肠道间质瘤的诊断价值。方法:对49例经手术病理证实为胃肠道间质瘤患者的64排螺旋CT影像表现及临床资料进行回顾性分析。结果:49例胃肠道间质瘤分别位于食道2例、责门区2例、胃24例、十二指肠1例、空肠12例、回肠4例、直肠1例、肠系膜2例、后腹膜1例。CT图像显示肿瘤可位于腔外、腔内或跨腔内外生长,形态呈类圆形或不规则分叶状,大部分边界较清,其内部可发生囊变、坏死、出血、钙化等,增强后肿瘤均匀或不均匀强化。结论:64排CT密度分辨率高,经重建后的图像可多方位显示肿瘤的位置、形态、生长方式、瘤内构成及毗邻关系,MIP血管成像还可以显示肿瘤供血动脉及与邻近大血管的关系,为肿瘤的定位、定性及确定治疗方案提供有价值的信息。Objective:To study the diagnostic value of 64-slice spiral CT to gastrointestinal stromal tumors. Methods: We retrospectively analyzed the clinical profiles and imaging presentations of 49 patients with pathology-confirmed gastrointestinal stromal tumors who underwent 64-slice spiral CT. Results: The 49 cases of gastrointestinal stromal tumors were located in the esophagus ( n = 2 ), cardia ( n = 2 ), stomach ( n = 24 ), duodenum (n=l), jejunum (n=12), ileum (n=4), rectum (n=l), mesentery (n=2) and posterior peritonea (n = 1 ). The tumors, presented as spherical-like or irregular lobulated mass on CT images, could survive extra-, intra- or transluminally with clear edges and cystic, necrotic or hemorrhagic changes or calcification interiorly, which could be homogeneously or heterogeneously enhanced. Conclusion: The 64-slice spiral CT, characterized by high resolution, may display the location, morphology, growth patterns, components and adjacent organs of tumors following image reconstruction. MIP angiography may demonstrate the tumor feeding artery and adjacent large vessels, thus providing valuable information for tumor localization, qualification and decision-making of treatment protocols.
