乳腺癌术后腋窝淋巴结新鲜标本的超声影像研究

摘　　要：目的探讨乳腺癌腋窝淋巴结转移声像图特征,评价超声对腋窝淋巴结转移的诊断价值。方法对17例乳腺癌改良根治术后腋窝淋巴结离体新鲜标本进行超声检查,对所探察到的97个淋巴结,在超声引导下行细针穿刺并编号标记,将标记淋巴结声像图与其病理结果作对应分析。按病理结果分为2组,淋巴结转移为阳性组46个,淋巴结无转移为阴性组51个。对比分析两组淋巴结二维声像图特征。结果转移性淋巴结灰阶超声表现为形态趋向圆形或不规则形,边界不光滑,短径>0.7 cm,长径/短径≤1.6,淋巴门移位或消失,皮质不均匀增厚,与阴性组差异均有统计学意义(P<0.001)。结论乳腺癌腋窝淋巴结转移的超声图像有特征性表现,灰阶超声多可做出正确诊断,超声对乳腺癌腋窝淋巴结转移的诊断具有重要价值。Objective To investigate the sonographic features of axillary lymph nodes metastases with breast cancer, and to evaluate the diagnostic value of ultrasound on axillary lymph nodes metastases. Methods The fresh specimens of 17 patients with breast cancer after modified radical mastectomy underwent ultrasound examination, and 97 detected lymph nodes were done the ultrasound guided fine needle wire localization and were numbered. Then sono- graphic features of every lymph node were analyzed and compared with corresponding pathology. The cases were divid- ed into two groups according to their pathological results： positive group of 46 cases, and negative group of 51 cases. Two-dimensional sonographic features were compared between the two groups. Results The two-dimensional ultra- sonography of metastatic lymph nodes showed round or irregular shape, unclear border, short diameter ~0.7 cm, as pect ratio ≤1.6, displacement or disappearance of hilus, and unevenly thickened cortical layer. There was statistical difference between the two groups （P-~0. 001）. Conclusions Ultrasound images of axillary lymph nodes metastases with breast cancer have characteristic features, and two-dimensional ultrasound could mostly make correct diagnosis. Ultrasound has important value in diagnosis on axillary lymph nodes metastases with breast cancer.
