宫颈鳞状上皮病变中Twist、Bcl-2蛋白的表达及意义

摘　　要：目的:研究Twist及Bcl-2蛋白在宫颈鳞状上皮病变中的表达及其意义。方法:选取正常宫颈上皮26例,宫颈上皮内瘤变(CIN)Ⅰ级标本42例,CINⅡ～Ⅲ级标本74例,宫颈鳞状细胞癌标本70例,运用免疫组织化学方法检测Twist、Bcl-2蛋白在各组织中的表达情况,并分析其与宫颈鳞癌的临床生物学行为的相关性。结果:正常宫颈组、CINⅠ级组、CINⅡ～Ⅲ级组、宫颈鳞癌组中Twist蛋白阳性表达率分别为0(0/26)、26.2%(11/42)、62.2%(46/74)和72.9%(51/70),Bcl-2蛋白阳性表达率分别为0(0/26)、19.0%(8/42)、43.2%(32/74)和68.6%(48/70),正常宫颈组、CINⅠ级组、CINⅡ～Ⅲ级组和宫颈鳞癌组的Twist及Bcl-2蛋白阳性率逐渐升高,4组阳性率差异均有统计学意义(P均<0.01);在宫颈鳞癌组中,Twist及Bcl-2蛋白在低分化肿瘤中的阳性率明显高于高、中分化肿瘤,ⅡB～ⅢA期阳性率高于Ⅰ～ⅡA期,有淋巴结转移者阳性率较无淋巴结转移者明显升高,差异均有统计学意义(P均<0.05);而不同年龄的患者比较,两蛋白阳性率差异均无统计学意义(P>0.05);Twist蛋白的高表达与Bcl-2蛋白的高表达呈显著相关(P<0.01)。结论:Twist和Bcl-2蛋白的表达在宫颈鳞状上皮病变的发生、发展中逐渐升高,两蛋白之间呈正相关,是临床早期发现宫颈鳞状上皮病变和判断宫颈鳞癌生物学行为有价值的标记物。Objective： To investigate the expression and s tein in cervical squamous epithelial lesions. Methods= A total of ignificance of Twist protein and Bcl-2 pro- 26 cases with normal cervical tissues, 42 cases with cervical intraepithelial neoplasis I （CIN I ）, 74 cases with CIN Ⅱ-Ⅲ, 70 cases with cervical squamous cell carcinoma were collected, the expression of Twist and Bcl-2 protein in cervical tissues was detected by immunohistoehemistry, and its relationship with clinical biological behaviors of cervical cancer was analyzed. Results=The expression rates Ⅱ-Ⅲgroup and cervical cancer group were 0 of Twist protein in normal control group, CIN I group, CIN % （0/26）, 26.2% （11/42）, 62.2% （46/74） and 72.9% （51/ 70）, the expression rates of Bcl-2 protein were 0% （0/26）, 19.0% （8/42）, 43.2% （32/74） and 68.6% （48/70）, respectively, the differences of four groups were statistically significant （P〈0.01）. Among cases with cervical squamous cell carcinoma, Twist and Bcl-2 protein positive rate in poorly differentiated tumor group was higher than high, moderately differentiated tumor group, the positive rate of lymph node metastasis was higher than those without lymph node metastasis, the positive rate of ⅡB-ⅢA period was higher than Ⅰ-Ⅱ A period, the difference were all statistically significant（P〈0.05） while no significant difference was observed between the different age groups （P〈0.05）. The high expression of Twist and Bcl-2 protein were positive correlated （P〈0.01）. Conclusions. Twist and Bcl-2 protein expression increa- ses gradually in the development of cervical squamous cell intraepithelial lesions, the two protein were pos- itive correlated and could be used as valuable biological marker to detect cervical squamous intraepithelial early lesions and determine biological behavior of cervical squamous cell carcinoma.
