胃肠道肿瘤化疗患者的疾病不确定感体验与应对方式的定性研究

摘　　要：目的:探讨胃肠道肿瘤患者化疗期间的疾病不确定感体验及其应对方式。方法:采用现象学研究方法,对安徽省某三甲医院就医的15例胃肠道肿瘤化疗患者进行半结构式深入访谈,现场录音和笔录,运用现象学7步分析法分析资料。结果:胃肠道肿瘤患者化疗期间的疾病不确定感体验包括,不确定症状的严重度及其意义;不明确治疗相关的各方面信息;对疾病相关知识与信息感到模糊;难以预测疾病预后;对各类社会支持的不确定。主要应对方式包括,寻求信仰;压抑、回避与发泄;渴望并寻求社会支持;主动寻求知识与信息,改变生活方式;调整与顺应,把握现在。结论:我国胃肠道化疗患者疾病不确定感体验与Mishel理论的观点基本吻合,同时具有文化特点,其应对方式包括情绪中心和问题中心两类。Objective：To understand the experience of uncertainty in illness and coping styles of gastrointestinal cancer patients during chemotherapy period. Methods： Fifteen patients with gastrointestinal cancer who were undergoing chemotherapy were selected from a hospital in Anhui Province.They were interviewed with a semi-structured interview format,and the data were recorded alive and analyzed with a phenomenological seven-step analysis method. Results： The experiences of uncertainty in illness of gastrointestinal cancer patients during chemotherapy period including ＂uncertainty of the severity and significance of symptoms＂,＂not clear about the treatment-related information＂,＂ambiguity of disease-related knowledge and information＂,＂difficult to predict disease prognosis＂,and ＂uncertainty about the various types of social support＂.The main coping styles including ＂seeking faith＂,＂repressing,avoiding and venting＂,＂being eager and seeking social support＂,＂taking the initiative to seek knowledge or information and changing lifestyles＂,and ＂adjusting to adapting,now or never＂. Conclusion： It suggests that the uncertainty experience of gastrointestinal cancer patients undergoing chemotherapy in China may be consistent with Mishel＇ s theory and has cultural characteristics at the same time.The coping styles include two types,namely,emotional-focused style and problem-focused one.
