摘 要:目的:探讨多发性骨髓瘤(multiple myeloma,MM)患者外周血中辅助性T细胞17(helper T cells17,17)细胞的检测及其临床意义。方法:收集39例MM患者及30例健康对照者的外周血,应用FCM法检测17细胞在CD4+T细胞中所占的比例,RT-PCR法检测17特异性转录因子维甲酸相关孤儿受体γt(retinoid-relatedorphan receptors-γt,ROR-γt)mRNA的表达水平,ELISA法检测血浆中白细胞介素-17(interleukin-17,IL-17)的表达水平。结果:MM患者外周血中17细胞所占比例、ROR-γt mRNA和IL-17的表达水平[(3.509±0.594)%、0.568±0.177和(27.200±4.666)pg/mL]均明显高于对照组[(2.913±0.846)%、0.468±0.163和(23.758±7.410)pg/mL](P均<0.05)。17细胞比例与MM患者的性别、年龄、疾病分型、球蛋白、免疫球蛋白、轻链、M蛋白和浆细胞比例无明显相关性(P>0.05),而与疾病国际分期系统(international staging system,ISS)的分期、β2-微球蛋白水平和疗效有关(P<0.05)。结论:17细胞可能参与了MM的发生和发展,检测17细胞的比例可能对判断MM患者的病情、疗效和预后有一定意义。Objective: To examine the Thl 7 cells (helper T cells 1 7) in peripheral blood of patients with MM (multiple myeloma) and explore its clinical significance. Methods: The peripheral blood samples from 39 patients with MM and 30 healthy volunteers were collected. The proportion of Th17 cells in CD4~ T cells was determined by flow cytometry. The expression of ROR-y/t (retinoid-related orphan receptor-,t) mRNA, a specific transcription factor of Thl 7 cells, was detected by RT-PCR. The plasma concentration of IL-1 7 (interleukin-1 7) was measured by ELISA assay. Results: The proportion of Thl 7 cells, the expression level of ROR-y/t mRNA and the concentration of IL-17 in peripheral blood in patients with MM were higher than those in the healthy volunteers [(3.509±0.594)% vs (2.913±0.846)%, 0.568±0.1 77 vs 0.468± 0.163, 27.200±4.666 vs 23.758±7.410 pg/mL; all P values 〈 0.05). The proportion of Th17 cells was not correlated with the gender, age, disease type, globulin, immune globulin, light chain, M-protein and the proportion of plasmocytes of the patients with MM (P 〉 0.05), but correlated with ISS (international staging system) stage, 132-microglobulin level and the therapeutic response (P 〈 0.05). Conclusion: Thl 7 cells may be involved in the occurrence and development of MM. Detection of proportion of Th17 cells may contribute to the ajudgement of disease severity, therapeutic response and the prognosis of patients with MM.
