小脑发育不良性节细胞瘤的影像诊断与鉴别诊断

摘　　要：目的分析小脑发育不良性节细胞瘤的CT和MRI表现,提高对其认识。方法回顾性分析经病理证实的12例小脑发育不良性节细胞瘤CT和MRI特点,分析病灶的大小、边界、内部特点、血供情况、占位效应及周围水肿等,并与9例小脑星形细胞瘤、10例髓母细胞瘤及10例小脑急性脑梗死征象进行对比分析。结果 "虎纹征"是小脑发育不良性节细胞瘤的特殊征象(特异性=100%,敏感性=100%);小脑发育不良性节细胞瘤中条状钙化征出现率(8例)明显高于髓母细胞瘤(1例)、星形细胞瘤(1例)及小脑急性脑梗死(0例)(χ2=5.091、4.411、7.793;P=0.011,0.024,0.002);小脑发育不良性节细胞瘤的占位效应与髓母细胞瘤、星形细胞瘤差别不大(q=1.009、2.019,P﹥0.05),与小脑脑梗死比较差异有显著统计学意义(q=6.912,P﹤0.01)。结论小脑发育不良性节细胞瘤的影像表现有一定的特点,对于鉴别小脑其他肿瘤或急性脑梗死有重要的参考价值。Objective to explore CT and MRI manifestations of dysplastic gangliocytoma of the cerebellum.Methods 12 patients suffered from dysplastic gangliocytoma of the cerebellum validated pathologically.Depending on the size,edge,interior,blood supply,space-occupying effect and peripheral edema of lesions,all data were analyzed retrospectively and compared with astrocytoma,medulloblastoma and acute cerebral infarction in cerebellum.The sizes of lesions were analyzed and compared.Results The Sign of tiger striping was the characteristics in dysplastic gangliocytoma of the cerebellum（specificity 100%,sensibility 100%）;the strip of calcification in dysplastic gangliocytoma of the cerebellum was higher than that of medulloblastoma,astrocytoma,and acute cerebellar cerebral infarction（χ2=5.091、4.411、7.793;P=0.011,0.024,0.002）.The space-occupying effect of dysplastic gangliocytoma of the cerebellum was similar to that of medulloblastoma and astrocytoma（q=1.009,2.019,P0.05）,but compared with acute cerebral infarction of cerebellum,there were the significant statistically differences（q=6.912,P0.01）.Conclusion Dysplastic gangliocytoma of the cerebellum has the characteristic appearances and can be better diagnosed by CT and MRI,and there is important value for distinguishing cerebellar tumors or acute cerebral infarction with dysplastic gangliocytoma of the cerebellum.
