仙方活命饮对大鼠肛周脓肿创面愈合实验研究

摘　　要：目的：探讨仙方活命饮对大鼠肛周脓肿创面愈合的影响。方法：60只SPF大鼠随机分为2组：治疗组予仙方活命饮汤剂配合常规用药，对照组单纯采用常规用药；制备大鼠肛周脓肿创面模型，给予药物干预后，分别于造模后第4、8、12、16、20天取创面肉芽组织，观察其病理变化，同时在造模后第2、5、10天测定周围血白细胞总数（WBC）、中性粒细胞（N）比例、血清肿瘤坏死因子d（TNF-a）、白介素6（IL-6）的变化。结果：在创面愈合早期（4～12d）。治疗组成纤维细胞数量大于对照组（P〈0．05）。在8d以后，治疗组的炎性细胞数少于对照组（P〈0．05）。治疗组可以显著降低WBC及N，缓解体内炎症状态，同时降低TNF-a及IL-6值更快、更显著（均P〈0．05）。结论：仙方活命饮对大鼠肛周脓肿创面肉芽组织的生长有明显的促进作用，且控制了创面的炎症反应，能显著促进创面的愈合，其有效治疗肛周脓肿症状的机制可能与其快速有效地降低TNF-a及IL-6这两个炎症细胞因子有关。Objective ： To investigate the promotion of Xianfang Huoming Yin on wound healing after rat＇s perianal abscess surgery. Method： Sixty SPF rats were randomly divided into two groups： treatment group and control group. Control group was given conventional treatment, and treatment group was given Xianfang Huoming Yin and conventional treatment. The pathological changes were observed after operation for perianal abscess and treated with drugs. The granulation tissue of wound was obtained on the 4th,8th,12th,16th,20th day. Values of white blood cell count, scale of neutrophilic granulocyte, serum tumor necrosis factor- at （ TNF- a）, interleukin 6 （ IL-6 ） in the 2 groups were compared as well as their clinical manifestations. Result： In the initial wound healing stage （4-12 d）, there were more blood capillaries and fibroblasts on the wound of treatment group than control group （ P〈0.05 ）. After the 8th day, there was less in inflammation cells on the wound of Xianfang Huoming Yin group in treatment group （ P〈0.05 ）. Comparing with the control group, the WBC, N, TNF-a and IL-6 were decreased more significantly and rapidly in the treatment group. Conclusion ： Xianfang Huoming Yin can promote growth of the granulation tissue and lighten the inflammatory reaction in the wound of the rat＇s abscess. Part of the mechanism of treating perianal abscess surgery with Xianfang Huoming Yin is related with the more rapid decrease of TNF- a and IL-6.
