胃窦溃疡患者幽门螺旋杆菌感染率与miR-21相关性研究

摘　　要：目的:研究胃窦溃疡患者幽门螺旋杆菌感染率与miR-21表达情况,分析两者的相关性。方法 :以2011年1-8月入住于广州军区广州总医院消化内科住院病区患者为研究对象,经胃镜检查及病理诊断为"胃窦溃疡"及"慢性浅表性胃窦炎"各120例。同时,留取胃窦病变黏膜组织及病变周围正常组织,采用实时荧光定量PCR技术分别检测组织miR-21表达水平。术后,各患者均进行13C呼气试验,明确是否幽门螺杆菌感染。结果:胃窦溃疡组的幽门螺旋杆菌感染率为80.83%,明显高于慢性浅表性胃窦炎组的56.67%(P=0.000);胃窦溃疡组的胃黏膜病变组织miR-21表达水平较周围正常组织高(P=0.000),慢性浅表性胃窦炎组的病变组织与周围正常组织miR-21表达无统计学差异;在胃窦溃疡组中,幽门螺旋杆菌感染患者与未感染者相比,其溃疡组织miR-21表达水平显著升高,差异有统计学意义(P<0.05)。结论:MiR-21高表达与幽门螺杆菌感染可能参与胃窦溃疡的发生、发展过程。Objective To study the relationship between the expression of miR-21 and the infection rate of Helicobacter pylori（Hp） in patients with gastric antral ulcer.Methods 120 patients with gastric antral ulcer and 120 patients with chronic superficial antral gastritis diagnosed by gastroscopy and mucosal biopsy were included in the study.Quantitative real time PCR was performed to detect the miR-21 expression in gastric antral ulcer tissue and lesion-adjacent normal tissue,and 13C urea breath test was used to investigate the Hp infection after operations.Results The infection rate of Hp in gastric antral ulcer group was 80.83%,which was significantly higher than that in chronic superficial antral gastritis group（56.67%,P = 0.000）.The expression of miR-21 in gastric antral ulcer tissue was higher than that in lesion-adjacent normal tissue（P = 0.000）.The expression of miR-21 in gastric antral ulcer tissue of patients with Hp infection was higher than that of patients without Hp infection（P 0.05）.Conclusion The high level of miR-21 and the Hp infection may contribute to the pathogenesis and development of gastric antral ulcer.
