摘 要:目的检测P16、Ki67、MMP2在CIN累腺、宫颈微小浸润癌(MIC)及宫颈鳞癌中的表达情况,探讨它们在宫颈CIN累腺与MIC鉴别应用中的意义。方法对正常宫颈对照组、各级CIN累腺组、MIC组及宫颈鳞癌组各20例,采用ElivisionTMsuper免疫组化二步法检测P16、Ki67、MMP2,并结合病理特征进行统计学分析。结果 P16、Ki67、MMP2在宫颈CIN累腺组与MIC组之间差异均有统计学意义(χ2=9.492,χ2=6.727,χ2=65.106,P<0.05)。且P16、Ki67在低级别CIN累腺组与高级别CIN累腺组间差异有统计学意义(χ2=47.473,χ2=23.177,P<0.05);而MMP2在高级别CIN累腺组与MIC组之间差异也存在统计学意义(χ2=47.473,P<0.05)。结论 P16、Ki67、MMP2三项联检能有效鉴别宫颈CIN累腺与MIC,指导临床。Objective To detect the levels of P16, Ki67 and matrix metalloproteinase 2 (MMP2) in cervical intraepithelial neoplasia(CIN) with gland involvement, microinvasive carcinoma of cervix(MIC) and cervical squamous cell cancer, discuss their value in the differential diagnosis between CIN with gland involvement and MIC of cervix. Methods The levels of P16, Ki67 and M MP2 of the normal control group( 20 cases) , CIN with gland involvement group (20 cases) , MIC group( 20 cases) and cervical squamous cell cancer group were detected by using Elivision Super Kit for immunohistoehemistry. The results combined with pathological characteristics were statistically analyzed. Results The levels of P16, Ki67 and MMP2 between CIN with gland involvement group and MIC group had significant differences ( ~2 = 9. 492,~2 = 6. 727, X2 = 65. 106, P 〈 0.05 ). There was sig- nificant difference in the levels of P16 and Ki67 between the lower grade CIN with gland involvement group and advance grade CIN with gland involvement group ( X2 = 47. 473, X2 = 23. 177,P 〈 0.05 ). Conclusion The combined detection of the levels of P16,Ki67 and MMP2 can effectively identify CIN with gland involvement and MIC of cervix.
