一次性鞘套胃镜结合上消化道症状调查表在边防官兵上消化道疾病诊断中的应用

摘　　要：目的评估一次性鞘套胃镜联合上消化道症状调查表对边防官兵上消化道疾病诊断的价值。方法 568例患者接受一次性鞘套胃镜检查并填写上消化道症状调查表。记录内镜操作时间和鞘套穿脱时间、胃镜检查结果、内镜操作过程中发生的问题,调查表按症状严重程度得出总分。结果一次性鞘套胃镜检查的平均操作时间为(4.9±1.4)min,鞘套穿脱平均时间为(4.9±0.2)min。一次性鞘套在穿脱、内镜操作过程中未发生破裂情况。胃镜下明显病变的总检出率为38.9%,最常见的疾病依次为胃炎(15.3%)、消化性溃疡(13.2%)和反流性食管炎(7.9%)。上消化道症状的严重程度与胃镜下是否有明显病变有显著关系(χ2=11.04,P=0.001)。结论一次性鞘套胃镜联合上消化道症状调查表在边远地区等特殊场合下对上消化道疾病诊断有独特价值。Objective To investigate the diagnostic value of a novel disposable sheathed gastroscope combined with upper gastrointestinal symptoms questionnaire in outlying regions. Methods 568 participants had their gastroscopy per- formed with a disposable sheathed gastroscope and filled out an upper gastrointestinal symptoms questionnaire. The dura- tion of the endoscopic procedure and taking-off and putting-on the sheath were mearsured. The results of gastroscopy and any problems presented during procedure were also recorded. The scores of questionnaire were recorded. Results Mean procedure time of sheathed gastroscopy was （4.9 ＋ 1.4） min. The mean duration of replacing the sheath was （4.9 ~0.2） min. No sheath leakage or rupture was seen. The total detection rate of lesions was 38.9% by sheathed gastroscopy. There was a significant relationship between the upper gastrointestinal symptom and lesions under gastrosco- py （X2 = 11.04, P = 0. 001）. Conclusion The disposable sheathed gastroscope combined with upper gastrointestinal symptoms questionnaire is useful for upper gastrointestinal diseases screening in outlying regions.
