代谢综合征与颅内外动脉狭窄的相关性分析

摘　　要：目的探讨代谢综合征(MetS)与颅内、外动脉狭窄的相关性。方法依据2007年《中国成人血脂异常防治指南》MetS诊断标准将736例年龄大于或等于50岁的住院患者分为MetS组(n=216)和非MetS组(n=520),应用CT血管成像(CTA)检查判断有无颅内、外动脉狭窄,比较两组患者颅内、外动脉狭窄情况的差异并分析MetS与颅内、外动脉狭窄的相关性。结果 MetS组患者颅内、外动脉狭窄的发生率均显著高于非MetS组(P<0.01)。单因素Logistic回归分析显示MetS与颅内、外动脉狭窄均显著相关(P<0.01)。多因素Logistic回归分析显示MetS仅与颅内动脉狭窄独立相关(P<0.01),而与颅外动脉狭窄无独立相关性(P=0.58)。结论 MetS是颅内动脉狭窄的独立危险因素,而不是颅外动脉狭窄的独立危险因素。Objective To investigate the correlation between metabolic syndrome（MetS） with intracranial and extracranial arte- rial stenosis. Methods According to the MetS diagnostic criteria in the China＇s Adult Dyslipidemia Prevention Guide（2007）, 736 inpatients with MetS aged 950 years were divided into two groups,the MetS group （216 cases） and the non-MetS group （520 ca- ses）. All patients underwent CT angiography to examine intracranial and extracranial arterial stenosis. The prevalences of intracra- nial and extracranial arterial stenosis were compared between 2 groups and the correlation between MetS with intracranial arterial stenosis and extracranial arterial stenosis was analyzed respectively. Results The prevalence of intracranial and extracranial arterial stenosis in the MetS group was significantly higher than that in the non-MetS group. Univariate Logistic regress analysis revealed significant correlation between MetS with intracraiaial arterial stenosis or extracranial arterial stenosis （P〈0. 01）. Multivariate Lo- gistic regress analysis revealed that MetS was independently correlated with intracranial arterial stenosis （P〈0. 01） rather than ex- tracranial arterial stenosis（P=0. 58）, Conclusion MetS is an independent risk factor for intracranial arterial stenosis and not asso- ciated with extracranial arterial stenosis.
