血清TSGF及其他TM组合联检在消化系统恶性肿瘤诊断中的应用价值  被引量:6

摘  要:目的:研究肿瘤特异性生长因子(TSGF)及其他肿瘤标志物(TM)合理组合在消化系统恶性肿瘤诊断中的应用价值。方法:采用自动生化法检测127例恶性肿瘤(肝癌、胃癌、结肠癌及胰腺癌)患者、60例一般疾病患者和85例体检健康者的血清TSGF水平,应用化学发光法分别检测血清AFP、CEA、CA19-9、CA125及CA50等,观察TSGF在消化系统恶性肿瘤患者血清中的水平变化规律,计算其阳性率;并与对应的其它TM进行对比分析,评价其诊断价值及意义。结果:肝癌组、胃癌组、结肠癌组及胰腺癌组患者的TSGF水平均明显高于一般疾病组、健康对照组(P<0.01),一般疾病组TSGF阳性率也略高于健康对照组(P<0.01);消化系统各种恶性肿瘤组患者血清TSGF的敏感性均高于其他TM;TSGF与其他TM联检在各恶性肿瘤组敏感性高达72.22%~100%,将TM两两组合,发现TSGF+AFP、TSGF+CEA、TSGF+CA19-9及TSGF+CA50分别在肝癌、胃癌、结肠癌和胰腺癌组中有最高的互补性;进行系列试验时阳性率较低。结论:TSGF辅助诊断消化系统恶性肿瘤的敏感度高于其他TM,并具有广谱性;TSGF与相关TM的联检能最大程度地提高肿瘤的检出率及特异性,对消化系统恶性肿瘤的筛查及诊断有重要应用价值。Objective To study the applicative value of TSGF and combined other tumor markers (TM) detection in patients with digestive system malignacies. Methods TSGF was measured by automatic biochemistry method and other TMs(AFP,CEA, CA19-9, CA125 ,CA50) were measured by electroehemiluminescence assay. The results in 127 patients with malignant tumor and 60 common illness group as well as the healthy control group were analyzed. Results The levels and positivity rate of liver cancer group, stomach cancer group, colon cancer group and pancreatic cancer group were obviously higher than those in common illness group and the healthy control group (P 〈 0.01 ), and the common illness group presented higher than the healthy ones (P 〈 0.01 ). The sensitivity of TSGF showed higher than the other TMs in the digestive system malignancies. The sensitivity reached to 72% - 100% when the combined detection of TSGF and the other tumor markers. Combination the tumor marker in twos, discovered that the complementarity expressed highest in the combination of liver cancer group, stomach cancer group, colon cancer group and pancreatic cancer group were TSGF + AFP,TSGF + CEA,TSGF + CA19-9 and TSGF + CA50. Proportion of positive of all TMs was low in series experiment. Conclusion The sensitivity of TSGF is higher than the most other tumor markers in the auxiliary diagnosis digestive system malignant tumor, and has broad spectrum; the combined detection of TSGF and other related TM can significantly improve the detection rate and specificity in diagnosis of malignancies and provides important applicative value in screening and diagnosis of digestive system mallgnacies.
