摘  要:目的探讨特异的早期血小板活化表面标志物CD41a-FITC和PAC-1-FITC与老年颈动脉粥样硬化(CAA)的关系。方法采用流式细胞仪检测30例CAA(CAA组)患者,平均年龄(77.5±8.5)岁,外周血早期血小板活化因子PAC-1-FITC和CD41a-FITC阳性表达率;采用免疫透射比浊法测定血中高敏C-反应蛋白(hs-CRP);生化自动分析仪测定血低密度脂蛋白(LDL)的含量;并与20例健康查体老年人(对照组),平均年龄(73.5±7.5)岁比较。以两组的CD41a-FITC、PAC-1-FITC与颈动脉血流各项指标进行相关分析。并将CRP和LDL与早期血小板活化标志物CD41a-FITC及PAC-1-FITC进行相关性研究。结果CAA组PAC-1-FITC、CD41a-FITC和CRP、LDL的阳性表达(分别为96.6±11.8、29.2±13.8、4.0±1.5、3.3±2.3)高于对照组[分别为84.5±11.7、21.3±12.7、3.0±0.5、3.0±0.8(P<0.05)];CAA组血小板活化标志物PAC-1-FITC和CD41a-FITC阳性表达率(35.3%,98.8%)高于对照组[分别为15.8%、97.6%(P<0.05)]。在PAC-1-FITC和CD41a-FITC与颈动脉血流相关研究中发现:CAA组左颈内动脉收缩期血流(Vd)与早期血小板活化标志物呈负相关性(P<0.05),CD41a-FITC与Vd、Vm、PI呈负相关(P>0.05)。结论老年CAA患者早期血小板活化标志物PAC-1-FITC和CD41a-FITC检测可评估血小板早期活化状态,标志物的增高可能与动脉粥样硬化脑血管病有关。Objective To investigate relationships between the specificity of early platelet activation surface markers(CD41a-FITC,PAC-1-FITC) and the carotid atherosclerosis formation in the elderly.Methods Using flow cytometry method to detect the positive expression rate of early platelet-activating factor(PAC-1-FITC,CD41a-FITC)of peripheral blood of thirty cases of carotid artery atherosclerosis(CAA) patients [average age(77.5 ±8.5) years];Immune turbidimetric method was applied to determine super-sensitivity C-reactive protein(CRP) in bloods;Biochemical automatic analyzer was used to determine content of low-density protein(LDL)in bloods;Twenty cases of healthy elderly [average age(73.5±7.5) years] was served as control.Two Groups of CD41aFITE,PAC-1-FITE with carotid blood indicators was statistically analyzed.Correlation between CRP and LDL level with early platelet activation markers(PAC-1-FITC,CD41a-FITC) were analyzed.Results The positive expression of PAC-1-FIT,CD41aFITC,CRP,and LDL(96.6±11.8、29.0±13.8、4.0±1.5、3.3±2.3)in CCA group was higher than those in the control group [84.5±11.6、 21.3 ±12.7、3.0 ±0.5、3.0 ±0.8(P 0.05)].The positive expression rate of platelet activation markers PAC-1-FITC,CD41a-FITC in typical CAA patients was 35.3%,98.8%,higher than those in the control group[15.8%、97.6% respectively,P0.05)].Investigation of correlation of PAC-1-FITC,CD41a-FITC to carotid blood flow founded that left internal carotid artery systolic blood flow(Vd)in CAA group was negatively correlated with early platelet activation markers(P0.05),while CD41a-FITC was negatively correlated with Vd、Vm、PIalthough not significant(P0.05).Conclusions Detection of early platelet activation markers of PAC-1-FITC and CD41a-FITC in elderly carotid atherosclerosis can be assessed by early activation status of platelet.The increment of markers can be as a monitoring index for early cerebrovascular disease.
