肺癌术后胸腔积液患者胸腔内置管并灌注顺铂治疗的临床研究

摘　　要：目的比较经皮胸腔内置管和常规胸腔穿刺抽液并胸腔内灌注顺铂治疗恶性胸腔积液的疗效,探讨胸腔内置管治疗恶性胸腔积液的临床应用价值方法将确诊为肺癌致胸腔积液的患者100例,随机分为观察组50例、对照组50例,观察组患者采用经皮胸腔内置管引流胸腔积液,对照组采用常规胸腔穿刺抽液方法引流胸水,两组均行胸腔内灌注顺铂化疗,每4 d一次,连续灌注3～4次,比较两组患者的治疗效果。结果应用胸腔内置管引流胸水,并应用顺铂进行胸腔灌注化疗,胸水有效控制率达70.00%,其痛苦小、节约费用、应用方便,且不良反应少。结论经胸腔内置管引流胸水并胸腔内灌注顺铂治疗恶性胸腔积液,疗效确切,应用方便,提高生存质量,值得推广。Objective To compare the efficacy of intrapleural cisplatin treatment through intrapleural catheter and Traditional Pleural puncture for patients with pleural effusion caused by lung cancer,and to investigate the clinical value of intrapleural catheter in the treatment of malignant pleural effusion.Methods 100 patients with pleural effusion caused by lung cancer were recruited.They were divided into 2 groups randomly： treatment group（group T,n=50） and control group（group C,n=50）.The patients of group T accepted the intrapleural cisplatin treatment through intrapleural catheter,but the group C by traditional pleural puncture.The treatment effects of them were contrasted.Results The effect of intrapleural cisplatin treatment for patients with pleural effusion caused by lung cancer could be improved through intrapleural catheter.It was convenient,less painful,low-cost,and with less adverse reactions.Conclusion The intrapleural cisplatin treatment through Intrapleural catheter is Worthy of widely used for patients with pleural effusion caused by lung cancer.
