外路鼻腔泪囊吻合术与鼻内窥镜下鼻腔泪囊吻合术疗效比较

摘　　要：目的探讨外路鼻腔泪囊吻合术(EXT-DCR)与鼻内窥镜下鼻腔泪囊吻合术(EN-DCR)的疗效及手术适应证。方法选取221例慢性泪囊炎或泪管鼻阻塞患者为研究对象,其中101例采取EXT-DCR术式,120例采取EN-DCR术式,对手术效果观察3个月至2年。结果 EXT-DCR术后3个月无流泪、流脓,泪道冲洗通畅101例;术后一年复发流脓者2例,泪道冲洗不通畅者6例;术后二年成功率94%。EN-DCR术后三个月2例复发流泪,无流脓,泪道冲洗不通畅,半年后复发流脓。12例术后一年复发流泪,泪道冲洗不通畅,其中3例复发流脓。术后二年复发流泪,泪道冲洗不通畅4例。术后二年成功率85%。差异有统计学意义(P<0.05)。结论 EXT-DCR较EN-DCR成功率高,复发率低,复发周期长。对慢性泪囊炎患者采取何种术式,应视具体情况而定。Objective To discuss the efficacy of dacryocystorhinostomy（EXT-DCR） between external under endoscopy and operation indication.Methods 221 cases patients with chronic dacryocystitis or nasolacrimal duct obstruction patients were taken EXT-DCR operation and EN-DCR operation,operation effect was observed for 3 months to 2 years.Results EXT-DCR after 3 months without tears,bleeding,101 cases of lacrimal duct flush patency;postoperative A year after recurrence was in 2 cases,irrigation of lacrimal passage obstruction in 6 cases;after two years the success rate of 94%.EN-DCR three months postoperatively in 2 cases of recurrent tears,no pus,lacrimal duct flush unobstructed,half a year after the recurrence of pus.12 cases of postoperative recurrence in tears,lacrimal duct flush is not open,3 cases of recurrence of pus.Two year recurrence tears,4 cases of lacrimal duct flush is not smooth.After two years of success rate of 85%（P0.05）.Conclusion EXT-DCR high success rate,low relapse rate,long period of recurrence.Patients with chronic dacryocystitis should be chosen according to the concrete situation operation method.
