摘 要:目的探讨声脉冲辐射力弹性成像技术在诊断乳腺小肿瘤中的价值。资料与方法纳入68例经病理证实的乳腺小肿瘤患者共71个病灶,应用声脉冲辐射力成像技术测量肿瘤声触诊组织成像模式下与二维模式下的面积比(AR)、声触诊组织量化模式下肿瘤内部及瘤缘处组织的横向剪切波速度,并与病理结果进行比较。结果 68例乳腺小肿瘤患者的71个病灶中,良性32个,恶性39个。良性肿瘤的平均AR为0.98±0.19,明显低于恶性肿瘤的1.74±0.48(t=8.48,P<0.05);良性肿瘤内部的横向剪切波速度为(1.92±0.43)m/s,明显低于恶性肿瘤内部的(5.69±3.00)m/s(t=7.02,P<0.05);良性肿瘤周边组织的横向剪切波速度为(1.19±0.34)m/s,低于恶性肿瘤周边组织的(1.96±0.95)m/s(t=4.34,P<0.05)。结论声脉冲辐射力弹性成像技术为乳腺小肿瘤的鉴别诊断提供了一种定量的弹性诊断方法。Purpose To explore the value of acoustic radiation force impulse imaging in the diagnosis of small breast tumors. Materials and Methods 68 pathologically confirmed small breast cancer patients with a total of 71 lesions were included, acoustic radiation force impulse imaging technique was used to measure the tumor area ratio (AR) under the virtual touch tissue quantification mode and two dimensional mode, and the transverse shear wave velocity of the tissue within and at the edge of the tumor under virtual touch tissue quantification mode was also measured, results were compared with pathological findings. Results In the total 71 lesions of the 68 small breast cancer patients, 32 lesions were benign and the other 39 were malignant. Benign tumors had an average AR of 0.98±0.19, which was significantly lower than that of malignant tumors 1.74±0.48 (t=8.48, P&lt;0.05); transverse shear wave velocity within the benign lesions was (1.92±0.43) m/s, which was significantly lower than that of the malignant lesions (5.69±3.00) m/s (t=7.02, P&lt;0.05); transverse shear wave velocity at the edge of the benign lesions was (1.19±0.34) m/s, which was lower than that of the malignant lesions (1.96±0.95) m/s (t=4.34, P&lt;0.05). Conclusion Acoustic radiation force impulse imaging technique provides a quantitative elastic diagnostic method for the differential diagnosis of small breast tumors.
