摘  要:目的研究精神分裂幻听患者的全脑白质细微结构的改变情况。方法应用西门子3.0TMR对A组32例精神分裂幻听患者及B组32例正常对照者进行扩散张量成像(DTI)扫描,采用基于纤维骨架的空间统计(TBss)方法分析2组全脑DTI的各向异性分数(FA)图,并使用基于非参数的随机排列检验(RPT)对2组FA数据进行统计学比较。结果与正常对照组相比,精神分裂幻听组左侧钩束、下额一枕束及左侧丘脑前辐射区的左侧额叶白质、左侧内囊前肢及左侧外囊FA值明显降低,差异有统计学意义(P〈0.05,TFCE-corrected)。结论本研究结果表明,左侧钩束、下枕一枕束及左侧丘脑前辐射区的白质纤维损伤,提示左侧额叶与左侧丘脑、左枕颞区的白质纤维连接异常导致了精神分裂幻听的发生。Objective To study the change of the brain white matter microstructure in auditory hallucinations in schizophrenia. Methods Diffusion tensor imaging (DTI) scans were performed on group A of 32 auditory hallucinations in schizophrenia and group B of 32 cases of normal controls by using Siemens 3.0T MR. The fractional anisotropy (FA) values measured from the whole brain DTI analyzed by tract based spatial statistics (TBSS). Then compared FA data of two groups by random permutation test (RPT), which based on nonparametric. Results Compared with control group, FA values in the auditory hallucinations of schizophrenia group significantly lower in left uncinate fasciculus(UF),left inferior fronto-occipital fasciculus( IFOF), and left anterior thalamic ra- diation(ATR}, left white matter of frontal lobe, left anterior limb of the internal capsule and left external capsule. Conclusion It seemed that the left UF, left IFOF and left ATR white matter fiber have been damaged, which suggests that the abnormal connec- tions between left frontal lobe, left thalamus and left occipitotemporal lobe may cause auditory hallucinations in schizophrenia.
