X线摄影、B超检查对不同乳腺实质类型恶性病变的诊断价值

摘　　要：目的比较不同乳腺实质类型中，X线摄影、B超榆查对恶性病变的诊断价值。方法以术后病理结果为金标准，收集术前均行X线摄影、B超检查的患者资料，采用X^2检验，对2种检查方法的诊断效能进行分析。结果经手术或穿刺，病理证实的患者94例，共检出病灶100个，乳腺恶性病变57个。（1）乳腺恶性病变X线摄影、B超检查灵敏度92．98％、89．47％。特异度90．70％、79．07％，假阳性率9．30％、20．93％，假阴性率7．02％、10．50％，-致率92．00％、85．00％。（2）不同乳腺实质类型中，X线摄影、B超检查对恶性病变检出率分别为：脂肪型乳腺71．9％、75．0％，少量实质型乳腺68．4％、68．4％，多量实质型乳腺51．6％、45．2％，致密型乳腺27．8％、50．0％。结论乳腺X线摄影对乳腺恶性病变的总体诊断效能高于B超检查，同时，X线摄影对恶性病变的检出率受乳腺实质类型的影响高于B超检查。Objective To study mammography,B-ultrasound examination in the diagnosis of malignant lesions in different types of breast parenchyma. Methods Preoperative data of mammography,B uhrasound inspection were colleered and two kinds of inspec- tion methods of diagnosis efficiency were analysed by X^2 test with the postoperative pathologic results as gold standard. Results 100 lesions confirmed by surgery or puncture were detected among 94 patients , Breast malignant lesions were found in 57 cases. For breast malignant lesions, mammography and B-ultrasound inspection sensitivity were 92.98% ,89.47% specificity were 90. 70%, 79.07 %, false positive rate were 9.30% ,20.93%, false negative rate were 7.02% ,10.50%, agreement rate were 92.00% ,85.00% respectively. For different types of breast parenchyma, mammography and B-ultrasound inspection of malignant lesion detection rate were as following： fat type of the breast were 71.9%,75.0%, small amount of essence type mammary gland were 68.4% ,68.4%, large type essence of mammary gland were 51.6%, 45.2% , dense type were 27.8%,50.0%, respectively. Conclusion The overall diagnostic efficiency of breast malignant lesions in mammography is higher than that in B-ultrasound examination , at the same time, the influence of the mammary gland essence type to malignant lesion detection rate in mammography is higher than that in B-ultra- sound inspection.
