摘 要:目的:应用锥形束CT(cone-beam computed tomography,CBCT)重建恒牙期安氏Ⅱ1错患者颞下颌关节骨性结构,比较上颌前突和下颌后缩者颞下颌关节的差异。方法:对恒牙期安氏Ⅱ1错上颌前突和下颌后缩患者各30例进行CBCT扫描,利用invivo5软件三维重建,测量并进行统计学分析。结果:两组左、右两侧颞下颌关节无统计学差异;两组间颞下颌关节在部分测量项目中存在显著性差异(P<0.05)。结论:两组左、右侧颞下颌关节骨性结构基本对称;上颌前突组髁状突在关节窝中位置相对靠前;上颌前突组较下颌后缩组髁状突粗、长,关节窝宽、深,关节结节较高。Objective: To reconstruct and measure the skeletal structure of temporomandibular joint(TMJ) in Class Ⅱ^1 malocclusion in the permanent dentition using cone-beam computed tomography(CBCT) and to compare the differences between maxillary protrusion and mandibular retrusion. Methods.. Permanent dentition on 60 cases of ClassⅡ^1malocclusion patients were selected for the study, 30 patients with anterior maxillary and mandibular retrusion in each group. The original images scanned by CBCT were reconstructed with invivo 5 software and then re- constructed three-dimensional images to located. Results: There was no statistically significant difference of the shape of TMJ between two sides incredibly in the two group. Some of the skeletal structure of TMJ were significant different between maxillary protrusion sample and mandibular retrusion sample (P〉0.05). Structures of TMJ were relatively symmetry between two sides of the two group. The condyles of the maxillary protrusion sample were more anterior than mandibular. Conclusion: The skeletal retrusion in the glenoid fossa. Compared with the mandibular retrusion sample, the condyle of the maxillary protrusion is wider and longer and the glenoid fossa is deeper and wider the condyle is thiner and shorter and the glenoid fossa is shallower and narrower in the other sample.
