小切口内固定治疗颧弓骨折46例临床分析

摘　　要：目的:探讨小切口在颧弓骨折内固定中的临床效果。方法:46例颧弓骨折患者经术前颌面部CT平扫+三维重建确诊后,全麻下取颧弓骨折凹陷处表面皮肤小切口,骨折复位后用微型钛板坚强内固定。结果:所有患者伤口均Ⅰ期愈合,无面神经损伤发生。术后12个月随访,4例术后张口度小于3.5cm的患者经过张口训练后,张口度恢复正常;3例术后6个月仍眶下区麻木的患者明显有好转。结论:颧弓内固定小切口治疗,患者手术创伤小,路径短,能有效恢复颧弓外形及开口功能,符合微创手术原则,值得临床推广。Objective： To evaluate the clinical effect of minimal incision in the treatment of rigid internal fixation for zygomatic arch fracture. Methods： Forty-six cases with zygomatic arch fracture were diagnosed with 3-Dimensional CT preoperatively. Under general anesthesia, minimal incision was used on the treatment of rigid internal fixation for zygomatic arch fracture. Results： Postoperatively, all patients＇ incisions were healed in stage I and none was occurred in the injury of facial nerve. During 12 months follow up, 4 cases with less than 3.5 cm in mouth opening was returned to normal through the rehabilitation treatment, and the feeling in 3 cases with the infraorbital area numbness was recovered in some degree. Conclusion： The minimal incision for the treatment of rigid internal fixation for zygomatic arch fracture makes the surgical trauma reduce and path shorten, with effective restoration of zygomatic arch shape and mouth opening function, which is in line with the principle of minimal invasive surgery and deserved to be recommended.
