乳腺浸润性导管癌在自动乳腺容积成像系统中声像图特征分析

摘　　要：目的探讨自动乳腺容积成像(ABVS)系统中乳腺浸润性导管癌(IDC)的声像图特征及其应用价值。方法回顾性分析41例患者47个经手术病理证实的IDC病灶在ABVS中的声像图特征。结果 47个IDC病灶中毛刺征47个(100%),恶性晕40个(85.1%),汇聚征31个(66.0%),导管征30个(63.8%),微小钙化29个(61.7%),脂肪组织改变35个(74.5%),Cooper's韧带改变30个(63.8%),乳房后间隙及筋膜改变25个(53.2%),声衰减18个(38.3%)。不同直径的病灶在汇聚征、恶性晕、导管征、脂肪组织改变及Cooper's韧带改变5项声像图特征中差异有统计学意义(P<0.05)。结论 ABVS对IDC病灶的恶性声像图征象的检出有一定优势,有助于IDC的诊断。Objective To investigate the characteristics of breast invasive ductal carcinoma （ IDC ） using automated breast volume scanner （ ABVS ） and its value of application. Methods The ABVS ultrasonographic characteristics of 41 cases with 47 IDC lesions which confirmed by surgery and pathology analysis were reviewed retrospectively. Results The ultrasonic features of 47 IDC lesions with ABVS were as follows： spiculation 47 （ 100% ）, malignant halo 40 （85. 1% ）, convergence symptom 31 （66.0%）, ductal sign 30 （63.8%）, microcalcification 29 （61.7%）, fat change 35 （74.5%）, Cooper＇s ligament change 30 （63.8%） ,retromammary cellular space and fascia change 25 （ 53.2% ） ,acoustic attenuation 18 （38.3%）. There was a significance difference among different diameter lesions in convergence symptom, malignant halo, ductal sign, fat change and Cooper＇s ligament change （P 〈 0.05 ）. Conclusion ABVS has a certain advantage in detecting malignant uhrasonographic characteristics of IDC lesions signs which is helpful for the diagnosis of IDC.
