摘 要:目的观察扩血管药物硝普钠联合小容量高晶体-高胶体混合液(HHS)对电击致室颤兔心肺复苏血流动力学的影响。方法将18只新西兰大白兔随机分为肾上腺素组(A组)、硝普钠组(B组)和硝普钠联合高晶体-高胶体渗透压混合液组(C组),每组6只。心肺复苏术(CPR)开始后即刻应用上述3种药物,观察复苏中及复苏后动物的生理指标及状态。结果 CPR30min3组的平均动脉压(MAP)、冠脉灌注压(CPP)比较差异有统计学意义(P<0.05)。3组血pH值、二氧化碳分压(PCO2)、氧分压(PaO2)在复苏前后各时间点,差异无统计学意义(P>0.05)。结论与传统的收缩血管药物肾上腺素相比,硝普钠联合高晶体-高胶体渗透压混合液能有效改善心肺复苏的平均动脉压、冠脉灌注压等血流动力学效果。Objective To investigate the effect of combined sodium nitroprusside (SNP) with hypertonic-hyperoncotic solution (HHS) on the hemodynamics in cardiopulmonary resuscitation (CPR) rabbits after cardiac arrest induced by transoesophageal-chest wall electric stimulation. Methods Eighteen rabbits were randomly divided into 3 groups: standard CPR group (group A), SNP-enhanced cardiopulmonary resuscitation (group B) and SNP with HHS-enhanced cardiopulmonary resuscitation (group C). These 3 different CPR methods were used at 2 minutes after the application of the basic life support. The physical indexes and conditions were also monitored. Results After 30 min of CPR, there was a significant improvement in the hemodynamic parameters (especially MAP and CPP) in the SNP animals (P〈0.05). No significant difference in blood pH,PCO2 and PaO2 was noted among the three groups before and after CPR (P〉0.05). Conclusion Compared with epinephrine,combined sodium nitroprusside with hypertonic-hyperoncotic solution could enhance cardiopulmonary resuscitation and improve the effectiveness of hemodynamics during CPR, especially in the mean arterial pressure and coronary perfusion pressure.
