摘　　要：目的:回顾分析了Straumann synOcta八角基台或纯钛个性化基台-LAVA结合氧化锆全瓷一体冠用于低位咬合单个磨牙缺失修复的临床效果。方法:采用Straumann软组织水平种植体结合RN/WN synOct或3M LAVA基台系统对113名患者的125颗低位咬合单个缺失后牙进行修复。在种植体植入后、修复完成时(基线)及完成后3个月、6个月、1年时进行临床检查及X线检查,并调查患者的主观满意程度。结果:观察期中所有种植体正常。24例全瓷冠出现崩瓷(19.2%),垂直距离小于3.5 mm组崩瓷率高于垂直距离大于3.5 mm组(P<0.05)。修复1年后种植体周围均出现牙槽骨吸收。26例患者认为下颌修复体的固位螺丝孔影响美观(37.14%),缺牙区垂直间隙小于3.5 mm的患者不满意率高于垂直距离大于3.5 mm组(P<0.05)。不同基台修复方式之间结果差异无显著性(P>0.05)。结论:对于垂直距离小于3.5 mm的低位咬合缺牙修复不能通过修复方式的改变来避免出现崩瓷或美观问题。Objective： To evaluate the clinical effects of two kinds of titanium abutment-zirconia ceramic restoration for replacing single missing molar with infraocclusion. Methods： 113 patients with 125 missing molars with infraocclusion in posterior region were treated by Straumann RN/WN synOct or 3M LAVA titanium abutment combined with Cercon zirconia ceramic restoration to replace the missing teeth. At the time of implantation, restoration completeness （baseline） , 3 months, 6 months and 1 year after restoration clinical examinations were carried out respectively. Results： No failure was found in all implants. Porcelain cracking were found in 24 restorations （ 19.2% ）, and more porcelain failures were observed in the cases with less than 3.5 mm vertical distance （ P 〈 0.05）. By means of X-ray, bone absorption was observed around every implants after 1 years, and there were no statically differ- ences neither between two prosthodontic designs（ P 〉 0.05 ）, nor between different implant lengths （ P 〉 0.05 ）. 26 restorations in mandible were described as not acceptable by patients, most occurred in the cases with vertical space less than 3.5 mm （ P 〈 O. 05 ） , and there was no significant difference between two kinds of restorations （ P 〉 0.05 ）. Conclusion： For missing tooth with vertical distance less than 3.5 mm, it is difficult to avoid the problems of porcelain cracking and aesthetic dissatisfaction.
