胆管癌组织中HMGA2的表达变化及意义

摘　　要：目的探讨HMGA2蛋白在胆管癌组织中的表达及其临床意义。方法采用免疫组织化学EnVision法检测71例胆管癌组织及20例正常胆管组织中HMGA2蛋白表达,分析HMGA2表达与胆管癌临床病理特征及生存预后的关系。结果胆管癌组织及正常胆管组织中HMGA2蛋白的阳性表达率分别为64.8%、15%(P<0.05)。中-低分化腺癌、淋巴结转移、Ⅲ～Ⅳ期的病例HMGA2阳性表达率明显高于高分化、无淋巴结转移及Ⅰ～Ⅱ期病例(P<0.05或P<0.01);Kaplan-meier生存分析发现HMGA2阳性表达患者生存期明显低于HMGA2阴性表达患者(P<0.01)。Cox多因素分析显示,HMGA2表达情况、肿瘤手术切除以及肿瘤TNM分期是胆管癌患者独立的预后因子。结论 HMGA2表达与胆管癌发生、临床生物学行为及生存预后有密切关系。Objective To investigate the expression of HMGA2 protein in cholangiocarcinoma and its clinical signifi- cance. Methods Immunohistochemistry EnVision method was used to detect the HMGA2 protein expression in 71 cases of cholangiocarcinoma tissue and 20 cases of normal bile duct tissue. The relationship between HMGA2 expression and clinicopathological parameters and survival were analyzed statistically. Results The positive expression rates of HMGA2 protein in cholangiocarcinoma and normal bile duct tissue were 64.8% and 15%, respectively （P 〈0.05 ）. The positive rates of HMGA2 were significantly higher in the cases of moderately-differentiated or poorly-differentiation adenocarcinoma, lymphatic metastasis and stage Ⅲ or stage 1V than those in the ones of well-differentiated adenocarcinoma, non-meiastasis of lymphonode and stage I or stage Ⅱ （P 〈 0.05 or P 〈 0.01 ）. Kaplan-Meier analysis showed that increased expression of HMGA2 was associated with decreased overall survival（P 〈0.01 ）. HMGA2 expression together with tumor surgical ablation and tumor clinical stage proved to be independent prognostic parameters in Multivariate Cox regression analysis. Conclusion The expression of HMGA2 protein might be closely related to the carcinogenesis, clinical biological behaviors and prognosis of cholangiocarcinoma.
