阿立哌唑与喹硫平、奥氮平治疗精神分裂症激越行为的随机对照研究

摘　　要：目的比较阿立哌唑联合喹硫平、喹硫平联合奥氮平、阿立哌唑联合奥氮平对精神分裂症患者激越行为的疗效。方法将在我院住院治疗的90例有激越行为的精神分裂症患者按分层分组法分为阿立哌唑联合喹硫平组(Ⅰ组)、阿立哌唑联合奥氮平组(Ⅱ组)和喹硫平联合奥氮平组(Ⅲ组)。治疗后,对三组患者进行PANSS-EC评分,并分析不良反应发生率。结果三组患者治疗后各时间点PANSS-EC评分均显著下降(P<0.05);其中治疗1 d后PANSS-EC评分:Ⅰ和Ⅲ组有统计学意义(P<0.05),Ⅱ和Ⅲ组有统计学意义(P<0.05);治疗1周后PANSS-EC评分:Ⅰ和Ⅱ组有统计学意义(P<0.05),Ⅱ和Ⅲ组有统计学意义(P<0.05)。三组患者在治疗3d、5d、1周、2周后疗效无统计学差异(P>0.05)。与Ⅱ组相比,Ⅰ和Ⅲ组在震颤、失眠、肌强直等不良反应有统计学差异(P<0.05)。结论阿立哌唑联合喹硫平或喹硫平联合奥氮平对精神分裂症患者激越行为的控制效果明显,对后续治疗有积极作用,可以作为精神分裂症激越行为的治疗方案之一。Objective To cmparis of therapeutic effects of aripiprazole and quetiapine, olanzapine, quetiapine com- bined with aripiprazole combined with olanzapine in schizophrenia patients with agitation. Methods A total of 90 cas- es with agitation behaviors in patients with schizophrenia were randomly divided into aripiprazole and quetiapine group （group I ） combined with aripiprazole, olanzapine group （Group II） and quetiapine and olanzapine （group m） by the stratified randomization method.After treatment,the PANSS-EC score were used oto analyze the incidence rate of ad- verse reaction. Results In the three groups, the PANSS-EC score were significantly decreased at each time point after treatment （P 〈 0.05）;At the 1st day after treatment, there were significant differences in I and m group on the PANSS-EC scores （P 〈 0.05）, but there were significant differences in II and III group （P 〈 0.05）;1 weeks after the treatment, there were significant differences in I and I group on the PANSS-EC scores （P 〈 0.05）,and there were significant differences in II and III group （P 〈 0.05）.The efficay showed no significant difference after treatment for 3 days, 5 days, 1 week, 2 weeks of treatment among the three groups （P 〉 0.05）.I and III group had significant difference in the tremor, myotonia, insomnia and other adverse reactions compared with group II（P 〈 0.05）. Conclusion Aripipra- zole joint with Quetiapine or with Olanzapine do all have good effect on schizophrenia , and they have a positive effect on the follow-up treatment. They can be used as schizophrenia agitated behavior.
