“滋补汤”治疗气血两虚型老年晚期结直肠癌临床观察  

Clinical Observation of Zibutang in the Treatment of Advanced Colorectal Cancer of Qi and Blood Deficiency Pattern in the Elderly Patients

摘　　要：目的观察"滋补汤"对气血两虚型老年晚期结直肠癌患者中医证候、生活质量(Karnofsky,KPS评分)、中位生存期(mOS)和免疫功能状态的影响。方法 98例气血两虚型晚期结直肠癌患者,随机分为治疗组50例,对照组48例。对照组予以西医对症支持治疗,治疗组在西医治疗基础上加用"滋补汤"。治疗结束后评价、比较两组患者中医证候、KPS评分和中位生存期。测定两组治疗前后外周血T细胞亚群、NK细胞计数。结果治疗结束后,治疗组中医证候和生活质量改善与对照组比较,差异有统计学意义(P<0.05);治疗组中位生存期(6个月)较对照组(5.5个月)延长半个月(P=0.008)。治疗组治疗后外周血T淋巴细胞亚群CD3+、CD4+、CD56+(NK)阳性百分率明显高于本组治疗前水平,差异有统计学意义(P<0.05)。结论 "滋补汤"能改善老年气血两虚型晚期结直肠癌患者的临床症状,维护免疫功能,提高其生活质量,延长生存期。Objective To observe the impacts of zibutang on the syndrome of traditional Chinese medicine, life quality（ Karnofsky, KPS score ）, median survival time （ mOS ） and immune function state in the elderly patients of advanced colorectal cancer of qi and blood deficiency pattern. Methods Ninety - eight elderly patients of advanced colorectal cancer of qi and blood deficiency pattern were randomized into a treatment group（50 cases）and a control group（48 cases）. In the control group,the symptomatic support of western medicine was applied. In the treatment group, on the basis of the treatment as the control group, zibutang was supplemented. At the end of treatment, the syndrome of traditional Chinese medicine, KPS score and median survival time were evaluated and compared in the patients between the two groups. The peripheral T cell subgroups and NK cell count were determined before and after treatment. Results At the end of treatment, compared with the control group, the improvements in the syndrome of traditional Chinese medicine and life quality were different significantly in the treatment group （P 〈 0.05 ）. The median survival time （6 month）in the treatment group was prolonged by half a month as compared with the control group （ 5.5 months ） （ P = 0. 008）. After treatment,the levels of T cell subgroup, CD3^＋ , CD4 ^＋ , CD56^＋ and NK positive percentage were higher apparently than those before treatment in the same group, indicating the statistically significant difference （ P 〈 0.05 ）. Conclusion Zibutang relieves the clinical symptoms, maintains immune function, improves life quality and prolongs life span in the elderly patients of advanced colorectal cancer of qi and blood deficiency pattern.
