不同类型肺癌脑转移瘤的多层螺旋CT表现

摘　　要：目的分析肺癌脑转移瘤颅内病灶的CT图像特点。方法选择肺癌脑转移患者55例,所有患者均接受螺旋CT检查,以病理学结果为标准,观察不同类型脑转移瘤病灶数量及在脑内分布情况,比较不同类型脑转移瘤单发病灶及多发病灶分布情况、强化特点及瘤周水肿情况。结果①共检出腺癌脑转移转移灶65处,鳞癌脑转移灶26处,小细胞癌脑转移灶6处,大细胞癌脑转移灶2处,腺鳞癌脑转移灶9处。顶叶脑转移灶最多,其次为额叶及枕叶。②单发26例,多发29例,其中腺癌多发病灶最多(34.55%),鳞癌单发病灶最多(23.64%)。③脑转移瘤以结节性强化比率最高,为52.73%;不强化比率最低,为3.64%。腺癌及小细胞癌以结节性强化为主,鳞癌以环形强化为主。④脑转移瘤瘤周水肿以重度水肿比率最高(50.91%),轻度水肿比率最低(9.09%)。结论不同病理类型脑转移瘤的螺旋CT图像具有各自的特征。Objective To analyze the manifestations of CT scanning in brain metastasis of different types of lung cancers.Methods Fifty five patients with brain metastasis were chosen in this study,all of them had performed spiral CT scanning.Their pathological results as the standard were compared with the lesions of different types of brain metastasis,distribution of single lesion and multiple lesions in brain,enhanced features and peritumoral edema.Results ① There were 65 lesions of adenocarcinoma with brain metastasis including 26 lesions of squamous cell carcinoma with brain metastasis,6 lesions of small cell carcinoma with brain metastasis.② Large cell carcinoma with brain metastasis had been seen in 9 lesions of adenosquamous carcinoma.The most commonly seen locality was parietal brain metastasis followed by frontal and occipital lobes.The distribution of metastatic tumors was as follows： single in 26 cases and multiple in 29 cases,and adenocarcinoma accounted for 34.55%,squamous cell carcinoma with single lesion accounted for 23.64%.③ The characteristic manifestations of brain metastasis in CT scanning were as follows： higher proportion（52.73%） of metastasis manifested as nodular enhancement,and smaller proportion（3.64%） manifested as no enhancement.The adenocarcinoma and small cell carcinoma manifested mainly as nodular enhancement,squamous cell carcinoma mainly manifested as ring enhancement.④ Among peritumor edema in brain metastasis,the peritumoral edema of brain metastasis in higher proportion（50.91%） manifested as severe edema,and lowest proportion（9.09%） as mild edema.Conclusion Different pathological types of brain metastasis have different characteristics in cerebral CT scanning.
