摘 要:目的探讨新型scheimpnug/Placido三维眼前节分析仪Sirius测量健康人群角膜厚度的可重复性,以及与A型超声角膜测厚仪测量结果的一致性。方法前瞻性研究。选取36名志愿者右眼作为研究对象,由同一检查者依次使用Sirius系统和A超进行测量。将Sirius系统获得的中央角膜厚度(Central corneal thickness,CCT)和最薄点角膜厚度(Thinnest corneal thickness,TCT)分别与A超角膜测厚仪测量结果进行比较,采用配对t检验分析差异性;Pearson相关分析相关性;Bland—Altman分析一致性。结果Sirius系统测得的CCT、TCT和超声测厚仪结果的均值分别为(533.06±34.21)μm、(526.25_+34.21)μm和(525.94+34.51)μm。CCT和TCT的变异系数均小于0.6%,Cronbach Alpha系数、组内相关系数均大于0.99,Sirius系统测得的CCT、TCT具有很高的重复性。CCT与超声角膜厚度差异有统计学意义(P〈0.05);TCT与超声角膜厚度差异无统计学意义(P〉0.05);两仪器具有显著的相关性,CCT和TCT与超声角膜厚度的相关系数r分别为0.984和0.987(P〈0.05)。CCT和TCT与超声角膜厚度具有较好的一致性,95%的一致性界限分别为(-5.2μm,19.4μm)和(-10.9μm,11.5μm)。结论Sirius系统获得的中央角膜厚度、最薄点角膜厚度具有很好的重复性,与A型超声测厚仪具有高度的相关性和一致性,在临床工作中,可以考虑相互替代。Objective To assess the repeatability of corneal thickness measurements in healthy eyes obtained with a new Scheimpflug/Placid imaging system (Sirius,CSO Inc.) and the agreement of the measurements with those obtained by Ultrasound Pachymetry (UP). Methods In this prospective study, corneal thickness was measured in 36 subjects (36 eyes) by a single operator. Three consecutive scans were acquired for each eye.The central corneal thickness (CCT) and thinnest corneal thickness (TCT) of the Sirius were compared to UP, respectively. Data were compared using paired t test, the Pearson correlation test and Bland- Altman plot. Results The mean values of CCT, TCT obtained from Sirius and UP were 533.06±34.21, 526.25±34.51,and525.94±34.5μm, respectively. BothCCTandTCT showed high intra-observer repeatability, with all intra-class correlation coefficients and Cronbach's alphas were more than 0.99, and yielded aCoefficient of variation (COV)ofless than 0.6%. Significant difference was found between CCT and UP (P 〈0.05);No significant difference was found between TCT and UP (P 〉0.05).And regression analysis showed a high correlation between the measurements obtained with both devices(r =0.984,P 〈0.05; r =0.987,P 〈0.05). The Bland-Altman Plots displayed relatively good agreement (95% limits of agreement were -5.2 to +19.4μm and -10.9 to +11.5μm) between CCT and TCT to UE Conclusions In healthy eyes, the repeatability of Sirius is good. The agreement of Sirius and UP is high, and Sirius is better than the UP in observing the whole corneal thickness change. Because of high agreement between these devices, the Sirius is a non-contact method that may be an alternative to UP for measurement of corneal thickness.
