摘 要:目的探讨牙周炎症对缺血性脑卒中患者高敏C反应蛋白(hs—CRP)和纤维蛋白原水平的影响。方法选取缺血性脑卒中伴中重度牙周炎患者(脑卒中伴牙周炎组)、单纯缺血性脑卒中患者(脑卒中组)、单纯中重度牙周炎患者(牙周炎组)及健康者(对照组)各20例。记录牙周临床指标包括牙周探诊深度、附着丧失、出血指数、菌斑指数,检测血清hs.CRP和纤维蛋白原的含量。结果脑卒中伴牙周炎组牙周探诊深度、附着丧失、出血指数、菌斑指数均高于脑卒中组、牙周组、对照组(均P〈0.05)。牙周炎组、脑卒中组、脑卒中伴牙周炎组的hs—CRP和纤维蛋白原明显高于对照组[hs—CRP:(2.7±0.8)、(3.2±0.5)、(5.9±3.2)mg/L比(1.4±0.8)mg/L;纤维蛋白原(2.72±0.20)、(2.91±0.54)、(3.24±0.20)g/L比(2.29-t-O.14)g/L],脑卒中伴牙周炎组又明显高于脑卒中组、牙周组,差异均有统计学意义(均P〈0.05)。相关性分析结果显示,hs—CRP、纤维蛋白原水平与牙周指数正相关(均P〈0.05)。结论牙周感染和炎症可能是缺血性脑卒中患者hs—CRP和纤维蛋白原水平升高的原因之一。Objective To investigate the effect of periodontal inflammation on serum levels of high sensitivity C reactive protein (hs-CRP)and fibrinogen in patients with ischaemic stroke. Methods Eighty subjects were included in this study, including 20 patients with ischaemic stroke and periodontitis (SP group), 20 patients with ischaemic stroke (S group), 20 patients with periodontitis (P group) and 20 healthy controls (H group). Clinical periodontal parameters including probing depth (PD), attachment loss (AL), bleeding index (BI) and plaque in- dex (PLI) were measured. The serum levels of hs-CRP and fibrinogen were tested. Results The average PD, AL, BI and PLI were elevated in SP group than those in group P, S, H (P 〈 0.05). hs-CRP and fibrinogen levels of P, S, SP group were significantly higher than those of H group [ hs-CRP: (2.7 ± 0.8 ), (3.2 ± 0. 5 ), ( 5.9 ± 3.2)mg/L) vs (1.4±0.8)mg/L;fibrinogen: (2.72±0.20), (2.91 ±0.54), ( 3. 24 ± 0. 20 ) g/L vs(2.29 ± 0. 14)g/L] (all P 〈0.05) , hs-CRP and fibrinogen levels in group SP were higher than those in group S and P ( all P 〈 0.05 ). hs-CRP and Fg levels were significantly correlated to PD, AL, BI and PLI ( all P 〈 0. 05 ). Conclusion Inflammation of periodontal tissues might be one of the reasons for the increased hs-CRP and fibrinogen in patients with ischaemic stroke and chronic periodontitis.
