摘 要:背景:我国克罗恩病(CD)的发病率日益增长,其伴随的疾病如贫血等正逐渐受到关注。目的:探讨CD患者贫血因素及其临床意义。方法:纳入2011年6月~2012年8月同济大学附属第十人民医院40例CD患者,比较不同蒙特利尔分型的患者之间血红蛋白(Hb)、血清铁、总铁结合力、叶酸、维生素B12水平,并分析克罗恩病活动指数(CDAI)、CRP水平与Hb、血清铁的相关性。结果:34例(85.0%)CD患者伴有贫血。不同蒙特利尔分型的CD患者之间Hb、血清铁、总铁结合力、叶酸水平无明显差异(P=0.2061,P=0.2449,P=0.8007,P=0.8804),而维生素B12水平差异有统计学意义(P=0.0072)。CDAI、CRP与Hb、血清铁均呈负相关(P<0.05)。结论:L1型和L3型CD患者存在维生素B12相关贫血。CD患者Hb、血清铁水平下降与疾病炎症活动度增加有一定关联。Background: The incidence of Crohn' s disease (CD) in China is increasing, and its accompanied disease such as anemia has received more attention. Aims: To investigate the anemia factors and their significances in CD patients. Methods: Forty CD patients from June 2011 to August 2012 at Shanghai Tenth People' s Hospital of Tongji University were enrolled. The levels of hemoglobin (Hb), serum iron, total iron binding capacity, folic acid, vitamin B12 between different subtypes of CD according to Montreal classification were compared, and the relationship of Crohn' s disease activity index (CDAI)/CRP with Hb/serum iron was analyzed. Results: Of the 40 patienls, anemia occurred in 34 patients (85.0%). No significant differences in Hb, serum iron, total iron binding capacity, folic acid were found between patients with different subtypes of CD according to Montreal classification (P =0.2061, P =0.2449, P =0.8007, P =0.8804), whereas significant difference was found in vitamin Bt2 (P =0.0072). A negative correlation was found between CDAI/CRP and Hb/serum iron (P 〈 0.05 ). Conclusiolls: Patients with type L1 and type L3 CD of Montreal classification have vitamin B12-associated anemia, and the decrease of Hb and serum iron is correlated with the increase of inflammatory activity.
