摘  要:目的:评估双能量动脉、门静脉、延迟CT三期对比增强扫描的碘叠加值与胃腺癌病理分级间的关系。方法:80例胃癌患者均在术前行胃部双能量CT动脉、门静脉、延迟三期对比增强扫描,通过双能量后处理软件得到病灶的三期碘叠加值,并与主动脉进行标准化,计算三期间标准化碘叠加值差值。将每例患者所获得的所有参数与其肿瘤病理分级进行对照分析。结果:具有不同病理分级的患者,其延迟期标准化碘叠加值、标准化延迟-动脉期碘叠加值差值及标准化延迟-门静脉期碘叠加值差值均不同,且差异有统计学意义(P=0.018、0.008、0.047)。多期双能量扫描的总有效剂量为(14.01±4.00)mSv。结论:双能量CT的碘叠加值可作为一种评估进展期胃癌患者预后的新指标。Objective To study the relationship between iodine overlay number (OLN) of triple-phase contrast- enhanced Dual-Energy CT and histological grading of gastric adenocarcinoma. Methods Eighty patients with gastric cancer underwent triple-phase contrast-enhanced gastric dual-energy CT (DECT) scan before surgery. The iodine overlay numbers were extracted by post-processing dual energy software. The normalized overlay number (NOLN) in the arterial, portal venous and delay phase (NOLNA, NOLNP and NOLND) and the normalized overlay number differences (NOLND) between the three phases were calculated. All of the parameters were analyzed in correlation with the histological grading of gastric cancer. Results Significant differences were found for NOLND, NOLND between arterial phase and delay phase and NOLND between portal venous phase and delay phase among patients with different histological grades (P=0.018, 0.008 and 0.047). The total calculated effective dose of muhiphase dual energy scan was (14.01 ±4.00) mSv. Conclusions The parameters derived from overlay number of DECT could be regarded as a potential novel prognostic indicator for patients with advanced gastric cancer.
