腮腺基底细胞腺瘤影像学表现与病理学对照研究

摘　　要：目的:探讨腮腺基底细胞腺瘤的影像学特点及其病理学基础。方法:回顾性分析经病理证实的13例腮腺基底细胞腺瘤患者的临床特征、影像学表现和病理结果,并将影像学表现与病理学结果进行对照研究。结果:病理检查结果,13例腮腺基底细胞腺瘤患者中7例为实性型,2例管型,3例小梁型和1例膜型。13例均为单发肿瘤,最大径小于3 cm;9例位于腮腺浅叶,4例位于腮腺深叶,突向浅叶;9例肿瘤为类圆形,3例为椭圆形,1例呈融合结节样。7例实性肿瘤增强后动脉期明显强化;2例囊性伴壁结节,增强后动脉期壁结节显著强化;3例实性伴囊变,增强后实性部分中等度强化;1例呈多发结节样改变,增强后结节明显强化。结论:腮腺基底细胞腺瘤好发于中老年女性,大多数呈最大径小于3 cm的单发类圆形肿块,其影像学表现与肿瘤的病理特征及分型相关。Objective To study the imaging features and pathology of basal cell adenomas （BCAs） of parotid gland. Methods Medical characteristics and images of BCAs with pathologic correlation in 13 cases confirmed by operation and pathology were retrospectively analyzed. Results On pathologic examination, 7 tumors were solid type, 2 were tubular type, 3 were trabecular type and 1 was membranous type. All 13 cases were single tumor with less than 3 cm in maximum diameter; 9 cases were located in the superficial lobe and 4 were in the deep lobe protruding to the surface; 9 tumors were round, 3 were slightly ovoid in shape and 1 was muhinodular confluent tumor; 7 tumors were solid-looking showing strong contrast enhancement; 2 were cystic masses contained mural nodules with obvious enhancement of the nodules; 3 were composed of central cystic and peripheral soft-tissue with moderate enhancement of the solid portion; 1 was composed of many nodules with significant enhancement. Conclusions Basal cell adenoma of parotid gland often occurs in old women, the feature is a small single lesion with less than 3 cm in maximum diameter, imaging features are related with pathologic characteristics and subtypes.
