肺及部分左心房切除治疗局部晚期肺癌的临床分析

摘　　要：目的探讨肺及部分左心房切除术治疗局部期肺癌的疗效。方法选取1990年1月至2008年1月间行肺及左心房部分切除治疗的56例局部晚期肺癌患者,其中左全肺加部分左心房切除31例,右全肺加部分左心房切除术7例,左肺下叶加部分左心房切除6例,右肺中下叶加部分左心房切除9例,右肺中上叶加部分左心房切除术3例。结果 56例患者中,围手术期死亡2例。1、3、5年生存率分别为81.5%、57.4%和20.4%,截至到随访日仍有3例存活。结论肺叶或全肺切除并部分左心房切除能提高局部晚期肺癌手术切除率,延长生存时间。Objective To investigate the value of partial atrium resection for the treatment of locally advanced lung cancer.Methods From January 1990 to January 2008,fifty-six patients with locally advanced lung cancer（T 4 N 0-N 2 M 0） underwent lobectomy or pneumonectomy combined with partial resection of the atrium.Of the 56 patients,31 underwent left pneumonectomy combined with partial resection of the left atrium,7 underwent right pneumonectomy with partial resection of the left atrium,6 received left lower lobectomy with partial resection of the left atrium,9 had middle and right lower lobectomy with partial resection of the left atrium,3 underwent middle and right upper lobectomy with partial resection of the left atrium.Results 56 patients were all operated.Two postoperative death occurred in the 56 patients.The 1,3 and 5 years survival rate of the patients were 81.5% 、57.4% and 20.4% respectively.Until the end of follow-up time,there were still 3 cases survived.Conclusions Pneumonectomy or lobectomy combined with partial resection of the atrium can enhance the possibility of radical resection of locally advanced lung cancer and increase the long term survival rate.
