摘　　要：目的探讨围手术期输血对胃癌患者术后复发及预后的影响。方法将152例接受胃癌根治术患者分为输血组(47例)和非输血组(105例),回顾性地分析围手术期输血与临床病理参数之间的关系及其对术后复发和预后的影响。结果围手术期输血发生率与患者肿瘤大小、肿瘤侵袭程度、淋巴结转移、TNM分期和有无术前贫血显著相关(P<0.05)。输血组患者术后复发率显著高于未输血组(P<0.001)。单因素生存分析显示,输血组患者术后3年总体生存率显著低于未输血组(39.0%:70.3%,P<0.001)。Cox多因素生存分析显示,围手术期输血可作为判断胃癌预后的独立因素(P<0.001)。结论围手术期输血与胃癌患者复发增多及预后不良密切相关,临床实践中应严格掌握输血指征、合理输血。Objective To investigate the influence of perioperative blood transfusion on the recurrence and prognosis for patients with gastric cancer undergoing gastrectomy.Methods 152 patients with gastric cancer were divided into transfused and non-transfused groups.The association between perioperative blood transfusion and the clinicopathologic data for these cases was analyzed retrospectively.The influence of perioperative blood transfusion on the recurrence and prognosis for patients was also evaluated.Results Perioperative blood transfusion was significantly related with tumor size,depth of invasion,lymph lode metastasis,TNM stage and preoperative anemia（P 0.05）.The rate of recurrence for patients with blood transfusion was significantly higher than those without blood transfusion（P 0.001）.The survival data showed that the 3-year overall survival rate was significantly lower in the transfused patients than the nontransfused（39.0% vs70.3%,P 0.001）.In multivariate Cox regression analysis,blood transfusion was found as an independent prognostic factor（P 0.001）.Conclusions Perioerative blood transfusion was closely associated with more recurrence and worse prognosis following gastric cancer surgery.It is advisable for us to appropriate transfusion maybe required.
