使用双极射频切除止血设备Habib 4X肝切除18例分析

摘　　要：目的 探讨双极射频切除止血设备Habib 4X在肝脏切除手术中的应用效果.方法 使用Habib 4X肝脏切除手术18例（腹腔镜肝切除1例次）.原发性肝癌患者9例（其中合并肝硬化3例）,胆囊癌3例,左肝内胆管结石,肝内胆管炎,左肝萎缩3例,巨大肝血管瘤3例.在开腹和腹腔镜肝切除手术中,使用手执式一次性射频消融设备Habib 4X,垂直插入肝脏,沿所需路线进行切割.在电极之间所产生可控的射频能量,对肝脏组织包括胆管和血管进行封闭,然后使用手术刀进行肝切除.结果 肝切除过程中均未行肝门血管阻断.术中平均出血量180ml（50～350ml）,无输血病例,无胆漏发生,平均手术时间225min,无围手术期死亡病例.结论 严格选择手术适应证的前提下,Habib 4X切除肝脏是安全的,可以作为肝脏切除技术选择.Objective To investigate the application effect of Habib 4X radiofrequency-assisted bipolar hemostatic device in liver resection surgery. Methods From February 2010 to August 2011,18 patients accepted liver resection with the device（ laparoscopic liver resection in 1 case） ,including 9 cases of primary, liver cancer（ combined liver cirrhosis in 3 cases） ,3 cases of gallbladder cancer,3 cases of the left intrahepatic bile duct stones, intrahepatic cholangitis and left liver atrophy and 3 cases of giant hepatic hemangioma. In the open and laparoscopic liver resection, a handheld disposable radiofrequency ablation device Habib 4X was inserted into the liver vertically and cut along the desired route. A controllable radio frequency energy generated by electrodes was used to seal the liver tissue, including the bile duct and blood vessels, and then the scalpel was used for liver resection. Results During the liver resection process,no hepatic portal occlusion was performed. The intraoperative mean blood loss was 180 m1（50 - 350 ml）. No blood transfusion or bile leakage occurred. The mean operative time was 225 minutes. There were no perioperative deaths. Conclusion Based on the premise of strict selection of surgical indication, liver resection by Habib 4X is safe,which could be a choice as liver resection technique.
