摘 要:目的探讨彩色多普勒超声、超声弹性成像及X线钼靶三者联合应用对乳腺肿块良恶性诊断价值。方法 2010年10月—2012年5月经手术治疗病理证实的乳腺肿块患者58例共67个病灶,在术前均行彩色多普勒超声、超声弹性成像及X线钼靶检查。各组间两两比较采用χ2检验,P﹤0.05为差异有统计学意义。结果彩色多普勒超声诊断乳腺肿块的敏感性72.2%,特异性83.6%,准确性80.6%;超声弹性成像诊断乳腺肿块的敏感性89.4%,特异性93.8%,准确性92.3%;X性钼靶诊断乳腺肿块的敏感性55.6%,特异性84.9%,准确性79.1%;三种方法联合应用敏感性94.4%、特异性97.9%、敏感性95.5%。各组间两两准确率比较,超声弹性成像组和联合组准确率均明显高于彩色多普勒超声组与X线钼靶组(均P﹤0.05);联合组与超声弹性成像组无统计学意义(χ2=1.357,P>0.05)。结论彩色多普勒超声、超声弹性成像及X线钼靶三者联合可显著提高乳腺良恶性肿块的诊断率。Objective To assess the value of Color Doppler ultrasonography combined with ultrasonic elastography and mammography in the differentiation of benign and malignant breast lesions.Methods From October 2010 to May 2012,58 patients with breast lesions who had been surgically treated and had 67 pathologically confirmed lesions were selected.Color Doppler ultrasonography,ultrasonic elastography and mammography were conducted before surgery.χ2 test was used for the comparison between groups,the result of P〈0.05 was considered statistically significant.Results For color Doppler ultrasonography,the sensitivity,specificity and accuracy were 72.2%,83.6% and 80.6% respectively;for ultrasonic elastography,they were 89.4%,93.8% and 92.3% respectively;for mammography,they were 55.6%,84.9% and 79.1% respectively;for the combined used of Color Doppler ultrasonography,ultrasonic elastography and mammography,they were 94.4%,97.9% and 95.5% respectively.For accuracy,both ultrasonic elastography group and combination group were significantly higher than Color Doppler ultrasonography group and mammography group(P〈0.05);no significant difference was found between ultrasonic elastography group and combination group(χ2=1.357,P〈0.05).Conclusions Combined use of Doppler ultrasonography,ultrasonic elastography and mammography can obviously improve the diagnostic accuracy of breast lesions.
