细胞因子诱导的杀伤细胞在早期肾癌术后治疗中的应用

摘　　要：目的观察早期肾癌根治术后行自体细胞因子诱导的杀伤(cytokine-induced killer,CIK)细胞治疗的临床疗效。方法选取解放军总医院2009年期间Ⅰ、Ⅱ期肾癌术后患者40例,治疗组20例采自体外周CIK细胞血进行CIK细胞扩增,采取静脉回输方法治疗;对照组20例术后未行其他治疗。观察CIK细胞培养前后细胞表型情况,对比治疗前及治疗后4周、24个月外周血T淋巴细胞亚群变化,记录不良反应情况、疗效和生活质量评定。结果 CIK细胞培养后可见CD3+CD8+、CD3+CD56+细胞较培养前显著增加(P<0.01);治疗后外周血CD3+、CD4+、CD4+/CD8+较治疗前升高(P<0.01)。治疗组20例:19例完全缓解,1例进展,患者治疗后生活质量提高,均无明显不良反应出现,疾病控制率95%;对照组20例:13例完全缓解,7例进展,疾病控制率65%(P<0.05)。结论自体CIK细胞治疗可以提高患者的免疫功能,延缓和阻止早期肾癌术后患者肿瘤的复发或转移,延长无病生存期,提高生活质量。ObjectiveTo observe the efficacy of autologus cytokine induced killer（CIK）cells in the treatment of renal cell carcinoma（RCC）. MethodsTwenty patients diagnosed as RCC in stage ⅠandⅡ of TNM from Januarary to december 2009 were divided into 2 groups,CIK cells therapy group and the control group.The phenotypes were identified before and after CIK cells cultured.We detected the levels of CD3＋,CD4＋,cell ratio of CD4＋/ CD8＋ before and after treatment.The efficiency and the clinical symptoms of the treated patients were observed. ResultsThe levels of CD3＋,CD3＋CD8＋, and CD3＋CD56＋ were increased after culturing （P〈0.01）.The immunity function of therapy group significantly increased after being treated with CIK cells （P〈0.01）.The quality of life was improved.In therapy group there were 19 patients with complete remission,1 patient with disease progression,the rate of disease control is 95%. In control group there were 13 patients with complete remission,7 patients with disease progression. The rate of disease control is 65%（P〈0.05）.No adverse event happened during or after the CIK cells treatment. ConclusionAutologous CIK cells in treating RCC patients had a acceptable curative effect that can enhance cellular immune function and prolong disease progression.
