后循环缺血患者血浆β-内啡肽水平及脑干听觉诱发电位变化的研究  被引量:1

The Changes of Plasma β-endorphin Levels and Brain Auditory Evoked Potential in Patients with Posterior Circulation Ischemia

摘  要:目的研究后循环缺血患者血浆β-内啡肽(β-EP)及脑干听觉诱发电位(BAEP)的改变。方法对154例经临床确诊的后循环缺血患者依据临床症状及CT/MRI检查结果分为后循环梗死组(78例)和后循环TIA组(76例),均行血浆β-EP测定及BAEP检查,并和120例健康对照组进行对比。结果后循环梗死组患者血浆β-EP水平[(253.89±48.28)ng/L]显著高于健康对照组[(182.94±34.57),P<0.01]及后循环TIA组[(213.43±58.11),P<0.01],同时后循环TIA组患者血浆β-EP水平也显著高于健康对照组(P<0.01)。BAEP检查显示后循环梗死组患者的Ⅲ波潜伏期[(4.12±0.13)ms]较健康对照组[(3.74±0.22)ms]及后循环TIA组[(3.98±0.21)ms]延长(P<0.05),后循环梗死组患者的Ⅴ波潜伏期[(6.04±0.25)ms]较健康对照组[(5.69±0.24)ms]及后循环TIA组[(5.75±0.39)ms]显著延长(P<0.01),后循环梗死组患者的Ⅲ~Ⅴ波潜伏间期[(2.08±0.23)ms]较健康对照组[(1.92±0.22)ms]及后循环TIA组[(1.77±0.31)ms]显著延长(P<0.01)。后循环缺血患者血浆β-EP浓度与Ⅴ波潜伏期和Ⅲ~Ⅴ波潜伏间期呈正相关,r值分别为0.625(P<0.05)和0.682(P<0.05)。结论β-内啡肽在后循环缺血的病理生理过程中起着重要作用,可作为临床病情监测的辅助指标。Objective To investigate the changes of plasma β-endorphin (β-EP)levels and brain auditory evoked po- tential (BAEP) in patients with posterior circulation ischemia(PCI). Methods 154 patients with PCI which been divided in infarct group (78 cases) and TIA group (76 cases) were evaluated by plasma β-EP and BAEP compared with controls (120 cases). Results The plasma ^-EP in infarction group(253. 89±48. 28) ng/L was significantly higher than control group (182. 94±34. 57 ,P〈0. 01) and TIA group (213. 43±58.11 ,P〈0.01), the plasma β-EP in TIA group was signif- icantly higher than control group (P〈0.01). The peak latency (PL) of Ⅲ wave in infarction group (4. 12±0. 23) was significantly prolonged than control group (3. 74±0. 22) ms,P〈0. 05) and TIA group (3. 98±0. 21,P〈0. 05) . The PL of V wave in infarction group (6. 04±0. 25) was significantly prolonged than control group(5. 69±0.24ms,P〈0. 01) and TIA group (5. 75±0. 39, P〈0. 01). The inter peak latency (IPL) of Ⅲ-V wave in infarction group (2. 08±0. 23) was significantly prolonged than control group(1. 92±0. 22ms, P〈0. 01)and TIA group (1. 77±0. 31, P〈0. 01). The plasma β-endorphin levels were positively correlated with PL of V wave (r=O. 625, P〈0.05)and IPL of Ⅲ-V wave (r= 0. 682, P〈0. 05). Conclusions β-EP could played a very important role on the pathological physiology process in PCI, and could be taken as an assistant laboratory marker for clinical assessment of PCI.
