宫颈上皮内瘤变患者宫颈环形电切术后切缘阳性的研究

摘　　要：目的:探究宫颈上皮内瘤变(CIN)患者宫颈环形电切术(LEEP)后切缘阳性及病灶残留情况。方法:选择2008年8月-2011年8月本院采用LEEP治疗后的CIN患者398例作为研究对象,研究引起LEEP术后切缘阳性的相关因素。结果:年龄>40岁的患者切缘阳性率(14.2%)明显高于年龄≤40岁的患者(7.0%),差异有统计学意义(字2=5.598,P<0.05);HPV阳性者切缘阳性率(11.5%)高于HPV阴性者(2.9%),差异有统计学意义(字2=4.585,P<0.05);病灶累及腺体者切缘阳性率(12.3%)高于未累及腺体者(5.0%),差异有统计学意义(字2=4.986,P<0.05);病灶多发者切缘阳性率(12.7%)高于单发者(6.7%),差异有统计学意义(字2=3.900,P<0.05)。结论:年龄>40岁、HPV感染、病变累及腺体及多点病灶都是引起LEEP术后切缘阳性率高的因素。临床中LEEP术时不同的患者应该选择不同的切除深度和范围,减少病灶的残留。Objective： To study the situation of positive incisal margin and residual lesions for cervical loop electrosurgical excision procedure in patients with Cervical intraepithelial neoplasia （ CIN ） .Method： 398 patients with CIN were as research subjects to study the relative causes of positive incisal margin after LEEP.Result： Positive margin rate （ 14.2% ） in patients with Age〉40 was higher than patients with Age ≤ 40 （ 7.0% ）, there was statistically significant （X2=5.598, P〈0.05 ） .Positive margin rate for HPV-positive patients （ 11.5% ）was higher than HPV-negative patients （2.9%）, there was statistically significant （x2=4.585, P〈0.05 ） .The positive margin rate for patients whose lesions were involving glands （ 12.3% ） was higher than patients whose lesions were not involving glands （ 5.0% ）, there was statistically significant （X2=4.986, P〈0.05 ） .Positive margin rate for patients with multiple lesions （ 12.7% ） was higher than patients with single lesion （ 6.7% ）, there was statistically significant （X2 =3.900, P〈0.05 ） .Conclusion： Age〉40, HPV infection, lesions involving glands and multi-paint lesions are the factors caused high positive rate for incisal margin after treatment of LEEP.Different patients in the clinical LEEP procedure should choose the different depth and scope of excision to reduce the residual lesions.
