多指标联合检测对原发性肝癌的诊断价值分析

摘　　要：目的探讨血清甲胎蛋白异质体L3(AFP-L3)、甲胎蛋白(AFP)、铁蛋白(SF)、β2微球蛋白(β2-MG)联合检测对原发性肝细胞癌(PHC)的诊断价值。方法采用电化学发光免疫分析法及AFP-L3亲和吸附离心管法检测健康者及PHC、乙型病毒性肝炎(简称乙肝)后肝硬化、乙肝患者血清AFP-L3、AFP、SF、β2-MG水平。结果 PHC患者血清AFP-L3、AFP、SF、β2-MG水平高于其他各组(P<0.05)。AFP-L3、AFP、SF、β2-MG单项检测对PHC的诊断灵敏度分别为75%、62%、42%、84%,特异度分别为97%、92%、68%、71%,联合检测的诊断灵敏度为94%。结论 AFP-L3、AFP、SF、β2-MG联合检测可明显提高PHC诊断率,AFP-L3检测可提高AFP<400μg/L的PHC患者诊断率。Objective To investigate the value of combined detection of alpha-fetoprotein heterogeneity L3（AFP-L3）,alpha-fetoprotein（AFP）,serum ferritin（SF） and β2-microglobulin（β2-MG） in the diagnosis of primary hepatocellular carcinoma（PHC）.Methods Serum AFP-L3,AFP,SF and β2-MG were detected in healthy subjects and patients with PHC,cirrhosis or chronic hepatitis.Results The diagnostic sensitivity of AFP-L3,AFP,SF and β2-MG in PHC was 75%,62%,42% and 84%,and the diagnostic specificity was 97%,92%,68% and 71%,respectively.Combined detection of these four indicators could increase the diagnostic sensitivity up to 94%.Conclusion Combined detection could improve the diagnostic performance in PHC,and detection of AFP-L3 could increase the diagnostic performance in PHC when AFP level is below 400 μg/L.
