氧化亚氮/氧气吸入镇静应用于牙科恐惧伴高血压患者的有效性和安全性

摘　　要：目的观察氧化亚氮/氧气(N2O/O2)吸入镇静应用于牙科恐惧伴高血压患者的有效性和安全性。方法选择2011年3月至2012年6月广东省人民医院口腔科就诊的恐惧伴高血压患者76例,随机分为试验组和对照组,每组38例,试验组患者在行为劝导联合N2O/O2吸入镇静情况下进行治疗,对照组采用行为劝导方式进行治疗。观察比较2组的镇痛效果、焦虑评分、Ramsay镇静状态评分、Frankl治疗依从性评分、不良反应比例及血压波动情况。结果试验组镇痛效果显著优于对照组(P<0.05),焦虑程度也显著轻于对照组(P<0.05);试验组Ramsay镇静状态评分及Frankl治疗依从性评分均显著高于对照组(P<0.05);试验组患者血压波动超出正常范围的例数也显著少于对照组(P<0.05)。2组患者不良反应发生率比较差异无统计学意义(P>0.05)。结论 N2O/O2吸入镇静应用于牙科恐惧伴高血压患者疗效显著,安全性高。Objective To observe the efficiency and safety of nitrous oxide/oxygen （ N2O/O2 ） inhalation for patients with dental fear and hypertension. Methods A total of 76 patients with dental fear and hypertension in dental treatment were selected from Mar. 2011 to Jun. 2012 in Guangdong General Hospital. The cases were divided into experimental group and control group randomly, and each group had 38 cases. The patients in experimental group were given N2O/O2 inhalation and behavior way of persuasion, and the patients in control group were only given behavior way of persuasion. The analgesic effect, anxiety score, Ramsay sedated state score, Frankl treatment compliance score, adverse reaction ratio and fluctuation of blood pressure were observed and compared. Results In the experimental group, analgesic effect was significantly better than that in the control group（ P 〈0.05） ,the degree of anxiety was significantly lower than that in the control group（ P 〈 0.05 ）, Ramsay sedated state score and Frankl treatment compliance score were significantly higher than those in the control group （P 〈 O. 05 ） , and the number of patients whose degree of fluctuation of blood pressure was beyond the normal range was more than that in the control group （ P 〈0. 05 ）. The adverse reaction ratio had no significant difference between the two groups （ P 〉 0. 05 ）. Conclusion N2O/O2 inhalation has significant clinical effects on the patients with dental fear and hypertension, and the application has high safety.
