肝纤维化无创扫描仪观察替比夫定联合复方鳖甲软肝片治疗慢性乙型肝炎肝纤维化的疗效

摘　　要：目的用肝纤维化无创扫描仪(FibroScan)评价替比夫定联合复方鳖甲软肝片治疗慢性乙型肝炎肝纤维化的疗效。方法选取60例慢性乙型肝炎患者,随机分为治疗组和对照组,每组30例,治疗组接受替比夫定联合复方鳖甲软肝片治疗,对照组单用替比夫定治疗,疗程1 a,观察2组血清肝纤维化指标变化、肝功能复常率和乙型肝炎病毒(HBV)-DNA转阴率,用FibroScan观察肝脏瞬时弹性值(LSM)的变化。结果治疗组和对照组HBV-DNA转阴率、肝功能复常率比较差异无统计学意义(P>0.05);2组肝纤维化指标降低程度比较差异有统计学意义(P<0.05);治疗组和对照组LSM值降低程度比较差异有统计学意义(P<0.05)。结论替比夫定联合复方鳖甲软肝片可以更有效改善慢性乙型肝炎患者肝纤维化,FibroScan是临床评价慢性乙型肝炎肝纤维化治疗效果的有用工具。Objective To evaluate the curative effect of telbivudine combined compound Biejiaruangan troche on liver fibrosis in chronic hepatitis B by FibroScan. Methods Sixty patients with chronic hepatitis B were selected and were randomly divided into treatment group and control group, with 30 cases in each group. The patients in treatment group were treated with telbivudine combined compound Biejiaruangan troche, and the patients in control group were treated with telbivudine only. The course of treatment was 1 a. The index changes of liver fibrosis, liver function recovery rates and negative conversion ratios of hepatitis B virus（HBV）-DNA in serum were observed and the changes of liver stiffness measurement（LSM） were observed by FibroScan between the two groups. Results The liver function recovery rates and negative conversion ratios of HBV-DNA had no significant difference between the two groups （ P 〉 0. 05 ）. The reduced degrees of liver fibrosis index and LSM had significant differences between the two groups （ P 〈 0.05 ）. Conclusion Treatment of telbivudine combined compound Biejiaruangan troche can more effectively improve liver fibrosis in patients with chronic hepatitis B, and FibroScan is a useful tool for evaluating the curative effect of liver fibrosis in chronic hepatitis B.
